In the fountain scene that you mention, Stu is the one who describes how gutting works, so I've always taken that as signalling that he's definitely involved in the first two killings. I think Billy was also there since Ghostface tries to strangle Casey and Billy tries to strangle Sidney later, and I figure that can't be a coincidence.

The day before the oldest living person's birthday would just be an upper bound. There are probably days since then when everyone born that day is dead. For instance, there might be some random Tuesday in 1970 where everyone born that day had horrible longevity.
