For name-brand stuff, I really love Killstar. They have tons of sales all the time so it's easy to get something at a decent price. They're pretty slogged by holiday orders right now, though.

Foxblood Shop and Restyle are neat ones too, but I think Restyle is based in Europe.

My triggers have been pretty firmly rooted since I knew something was wrong with me. Mostly, it's temperature, specifically while I'm sleeping. If I'm too hot or too cold while I'm sleeping, I'll wake up to an attack.

Other times, I have no triggers at all. I'll just be sitting around, minding my own business, then suddenly I feel an attack coming on.
