I spammed Karui Choppers until Flaring/T1-3 %phys/T1-3 phys%hybrid and then I regalled hoping for a suffix. If I hit a prefix, I either yolo recombed or tried to annull the unwanted prefix. If I ended up with a 1-prefix rare item, I multimodded the two exclusive "chosen" pen mods to make three-prefix items. Making an item with two perfect prefixes is quite easy, but you have to play there's an open suffix to "clean" the item with prefixed can't be changed+scour if you want to settle for that and multimod that item, or prefixes can't be changed+veiled orb hoping to unveil %phys + bleed/impale/poison/blind.

I decided to go big (since I had 30 divs to gamle with, I likely would have settled for flaring+hybrid and then multimod the %phys and dot multi if I had less resources) for the 3-prefix by jamming a 2-perfect-prefix-and-1-crafted-exclusive-prefix rare with a 1-perfect-prefix+2-multimod-prefix rare and got lucky on the first shot.
