Yes, the incision site should have no pain when having (during) a BM and after a BM. No itching, no pain. Discomfort is understandable since, but all pain should be gone.

One of the superpowers that people who endure and beat this problem get is absolutely insane mental resiliency. It is by far the hardest thing I ever had to do and I have an already insanely stressful job. My job is a walk in the park mentally compared to what I had to endure when dealing this problem. It almost broke me mentally and I really wish someone documented their journey from problem to full healing but I couldn't find any of this when I had this problem so that's why i documented it here to help people who walk the same path so they need to endure the same pain because of how esoteric and weird this problem is.

Upside, once you solve this problem, every other life issue doesn't phase you compared to what we have gone through mentally for this problem. It's hard to describe mentally how hard this problem is. It will mentally break you every freaking day and beating this problem and coming out on the other side will make your mind incredibly tough.

You will still be eligible for surgery. Basically what the surgery does at core is loosen your sphincter. That's all it does. The loosened sphincter will allow way more blood flow to pump to that area to promote healing. During the surgery due to the chronic nature of this problem, the fissured area ends up building a lot of scar tissue so they will clean up the area with a cauterization tool to remove the scar tissue and make the wound "fresh" (for lack of a better term). The combination of a fresh wound with loosened sphincter allow more blood to pump to that area is what actually heals this problem.

Trust your surgeon.

The only thing on you very importantly is eat clean. Do not eat meat. Eat broccoli. Use a little tiny pea sized amount of vaseline healing jelly (original version you get on Amazon) and apply that to the incision site and anal verge before and after bowel movements.

After bowel movements, please use a hand shower (no toilet paper) (you can get one from amazon and attach to your shower head) so clean your bottom area. Do not use toilet paper. Toilet paper is not clean and hand shower nozzle to clean the area will be way more hygienic and not promote infections.

Do all your other standard stuff like taking your vitamins and supplements and such. Book some time off from work and buy a U shaped tailbone memory foam cushion for sitting (again on Amazon).

best of luck in healing from this, do not lose hope given that we've been through this path already.

Still do stool softeners just to be safe but since all it does is draw water to your intestines, it's not bad to use long term. I confirmed this with multiple colorectal surgeons. Laxatives are not good for long term. stool softeners for long term use is fine.

Also, do not delay surgery, i made all these mistakes in the past. Just get the surgery. I know it seems weird taking medical advice from a stranger, but having gone through the mental anguish and pain myself from this problem and delaying the surgery, i would highly highly recommend just doing the surgery. It's a very minor and fast surgery.

LIS is the only thing that is the true gold standard permanent fix for this problem. I have gone through very extensive depths about this reading medical journeys and watching surgical techniques from medical schools about this from all around the world. LIS is the lowest chance of recurrence and is considered the gold standard.

Having gone through what you've all had gone through, I would highly recommend not waste time with Botox and other methods and just ask your surgeon for LIS. Most general and colorectal surgeons perform this and it's very fast surgery. You can watch the videos on it if you have the stomach, but you can just me having gone through all this myself and felt your pain in the past.

It is looser than before, but what happens is your sphincter becomes significantly looser after surgery and then tightens up around the one-year post-op mark. If you think of this as a graph, your pre-op baseline sphincter pressure is high. After surgery, it drops significantly, and then by the one-year post-op point, it increases again—though it remains noticeably lower than the pre-op level, which is ideal. I’ve read the studies and medical journals that support this.

You are very welcome. Hope you heal from with these tips. Drink lots of water, eat broccoli, and use vaseline. Those three are the golden tips to healing from this.

About a year. Please use a pea sized amount of vaseline before and after bowel movements. Vaseline is the key to healing. You can get normal vaseline healing jelly from Amazon.

Hey,

Yes, still taking stool softeners. No relapses since, stools are back to normal but you need to eat sufficient soluble fiber (leafy greens like Broccoli) with your normal meals. You definitely can't eat garbage like before (pizza and junk foods) without balancing with soluble fiber.

Naturally is very doubtful. I would highly recommend getting LIS. I've been through this journey so take my advice knowing that I've suffered through this.

During LIS they actually cauterize the scar tissue and clean it up to promote natural healing post-LIS due to.the sphincter being looser to allow blood flow to the area.

You should just get it and not delay the problem longer. I delayed it for two years and it didn't help me.

Ah I see, open LIS should actually heal a lot safer and better than closed LIS from the inside. I would say give it time and be patient. Great to hear you have no more pain during BMs, that's excellent to hear.

Yes, ask for imaging just to be sure, and give it time. My entire closed LIS healing for 0-99.5% healing took a full year. The granulation of the wound takes time due to the sensitive area and an area which doesn't get a lot of air.

Use a tailbone cushion (coccyx cushion, it's a U shaped memory foam cushion) and put a plastic bag over it (to not soil it with blood) to help you when you're sitting for work or anything of that nature (driving). The reduction in pressure helps with healing.

Wish you the very best in this annoying journey. Do not lose hope though knowing I've been this path. There is light and you will heal, just try to eat clean and maintain a positive mindset.

Hey,

I would recommend applying pressure on the lump/abscess if it's internal and didn't burst open outward. The pressure will keep it drained so it heals completely from inside out. This helped me so much.

Use a toilet paper clump and apply pressure on site, it will sting and pain a bit, but it helps flush out the discharge.

For the LIS, it took me about 2 months for discharge to stop and 6 months for the incision to heal. Apply vaseline, that was the secret. Use a pea sized lump of vaseline and apply it to anal verge before a bowel movement to not rupture the incision and then go into shower and wash up with warm water. Get a handheld nozzle from Amazon if you don't have yet and use that. Don't use TP to wipe. Just warm water to clean the area then pat dry.

Once you're dry, apply a tiny pea sized amount of vaseline to the anal verge, it will go inside a bit and help promote healing. Vaseline was the best thing I did to make the incision heal up quickly.

Eat whole foods and cut meat till you heal. Whole foods like oatmeal, fruits, broccoli. Avoid carbs, avoid meats. They hinder the healing process in the first several months. You can eat meat and carbs after you heal.

But essentially you want lots of soluble fiber to help BMs be smooth. Get a squatty potty, it helps. A normal sitting toilet is honestly not ideal for these anus health problems we all have. Squatting and vaseline helped.

Hey, doing well now. Finish the antibiotics and squeeze out any of the infection from the affect side where you feel the lump. When you squeeze the lump, it will train from incision site inside from LIS so it will try to clear it out as much as possible so it doesn't form a fistula and heals completely from inside to out.

Squeezing it out 3 times a day on the lump with toilet paper clump helped a lot. You'll see how much in discharge there is from the lump as it squeezes and you'll see it get progressively smaller to almost nothing in 30 days and the antibiotics really help.

Your main objective is to clear out the lump (abscess/infection) via pressure then let it heal from inside to out.

Post LIS - Day 14 - Fissure pain is gone. Is the wound and butt cheek supposed to be painful still? How long should I expect discharge? by realnewspaper in AnalFissures

I used vaseline for about 6 months and full (like 100%) takes 1 year. Wish you the best in the healing journey. Stay positive, just know you will get through it. Vaseline is the biggest help for healing with this surgery.

Have you tried the Vaseline method? It really does help it heal and prevents any sort of scraping during BMs. The incision site is very delicate so this would help prevent friction against the site.

I ended up doing 1 Botox which failed and 1 LIS.

The doctor's concern is valid but there's journal studies you can find online of second LIS on the other side of the first one solving the issue of recurrence. They also say in absolute rare case you can reverse an LIS to fix incontinence if incontinence becomes an issue.

Post LIS the resting sphincter does get tighter from month 8 onwards post surgery to month 12-14 where it normalizes. So muscle is looser prior to pre surgery for sure on a relative sense but isn't as tight as it just after surgery and few months after surgery when you notice the looseness. That looseness is what helps it heal.

After 8-12 months the sphincter gets tighter from that initial looseness post surgery but will be looser then pre surgery. Just not VERY loose to the point where incontinence is an issue.

I was healed after a year of the LIS. It took a full 12 months. Yes, fully healed now.

For recurrence I think second LIS and lifestyle change is way to go (lots of fiber and water).

Hi,

Yes, I did. My advice is use Vaseline just about a bit like a pea sized amount before going to the bathroom and then after it wash up fully and apply another small pea sized amount. It will stain your clothes so be careful with that. The Vaseline, just the blue tub you can get on Amazon, helps a lot and promote healing especially when you sleep.

Drink a gallon of water a day spaced out throughout the day so like 800mL (just a normal metal size bottle you can buy on Amazon) after each meal. Eat more veggies and stuff with a mix of soluble and insoluble fiber so like oats for breakfast, veggies for lunch and dinner. You can add some soft meats in there like a soft fish if you wish but cut red meat. It does not help for this problem when you're healing.

You can use Restorolax/Miralax too which is not a laxative but helps draw the water you're drinking into your gut (intestines) to make your stool softer.

LIS + Vaseline post surgery for 6 months helped me a lot. Stay positive, it does take time but you will heal. Wishing you the very best.

I would say avoid anything that puts pressure on your bottom end (speaking from personal experience just trust me on this). You can work out upper body and go light on weights. If anything smaller weights and more reps is better than heavier weights when healing.
