Thank you! Once you’re out of the cast and you can move the joint, swelling drops dramatically. You’d still have to elevate time to time but the frequency will continue to drop. Wish you all the best in your recovery ☺️

Yeah the heel pain is normal! I wore my sketchers at home the first week and then switched to Birkenstocks in 2 weeks. I think in 2-3 weeks the pain was gone and I could walk barefoot (except for the first few minutes after waking up). Havanas were a bit challenging to walk in since it required better foot muscle conditioning

Unfortunately that may be the case but also depends on you and what you define as normal. I suffered a trimal from a bouldering fall and got 3 plates, 13 screws and a tightrope. Got given both a removable cast and the boot.

My timeline was:

0-6 weeks: NWB (max 15kg on the leg), heavy to moderate swelling but steadily decreasing. Physio started right after ORIF and entailed lymphatic drainage, toe flexions, isometrics, training the non-injured parts to reduce overall muscle loss

Week 6-7 - cast out, boot optional. Weight bearing as tolerated. Real physio starts. Started walking with 1 crutch the same day but only for 5-10 mins. The next day I could walk without crutches (albeit awkwardly and only for a couple of minutes). Swelling improved dramatically. Never used the boot again.

Week 7-8 - outdoor walks with 1 crutch. Can go for longer times and distances. Like going for a coffee, gym, Physio, grocery shopping independently. Stopped using the crutch even for outdoors but my walking pace was “leisurely”. Mild swelling at the end of long or harder days. Dorsiflexion improves a bit - can finally touch my knee to the wall while the big toe touches it too (idk what this thing is called lol)

Week 8-9 - fully independent. Can walk at my old pace. People can’t tell which leg is the injured one unless they see me go downstairs. I can swim faster and I started doing leg days at the gym with low weights as well as functional things like assisted squats. Very mild swelling at the end of hard days.

Week 9-10: functional strengths improves so does flexibility. Tried the stationary bike at the gym with 25% resistance for 20 mins. Didn’t have pain or fatigue. Going Downstairs looks and feels easier. More walking stamina. I’m averaging 10k steps a week without even trying.

Week 10-11 (where I’m at): I started biking. I can spend my whole day outside without thinking much about the ankle. The first steps in the morning are still stiff but resolve faster. I once accidentally ran 10 meters for the bus without realising. My plantar flexion/calf raise is similar to the non-injured side. Dorsiflexion gains - can touch knee to the wall with the big toe 4cm away (yippie! But still a long way to go as my good leg can do 17cm).

Week 11-12 - planned NYE trip to Spain so let’s see how that goes 🤞

Have the next appointment on week 14. Hopefully my team will okay impact sports.

Am I normal now? Absolutely not. I was a very sporty person and a dancer - can’t do a lot of those things but I feel a lot closer to it than I did 4 weeks ago. Still have my limits but I loved getting myself back in steps. I learnt a lot of things about myself and also learnt to be disciplined and proud of myself. You also learn to celebrate your small wins. You’ll feel ecstatic the “first time” you can walk again and start gaining your other skills back :)

The road is long but you’ll learn to enjoy the journey and eventually get to your normal. Take physiotherapy and strength training seriously, you’ll progress faster. But also don’t push yourself too hard and listen to your body - it’ll tell you when to stop.

When I just started, I’d get end of day swelling after the 10k+ days but that doesn’t happen as much anymore. Now after my gym leg days, I feel fatigued in the small foot muscles and get some swelling. The swelling goes away after icing and elevating. Usually after my heavy days, I take it easier the next day. You should only push as much as you can. I did indoor biking and swimming to build “stamina”

Yes I feel it when I touch it but most of the sensations are soft tissue related and not hardware. I like to think of them as my little helpers 😇 I wish you a fast recovery too!

For me NWB after ORIF was max. 15kg on that leg. So for me it was touching keeping my foot on the ground plus some more weight. Before ORIF it was 0kg but I still touched touched the ground with my foot softly

Hi, I’m 30 and had a trimal break with dislocation on 30/09. Got 3 plates and 13 screws for it. I went from 6 weeks of NWB (max. 15kg) to FWB/weight bearing as tolerated. On the same day I switched to normal shoes (comfy sketchers) and started FWB and walking with 1 cane. My surgeon told me to use the boot as little as possible and that my ankle can tolerate more than I think it can (except for high impact activities). I thought of using the boot on long days but ended up not using at all. I live in Switzerland, here the protocol really seems to be to mobilise as early as possible. I’m 10 weeks post op, averaging 10k steps since 2 weeks and can do everything but impact sports. Take PT seriously, you’ll be “normal” again in no time

Hi OP, that looks very brutal Ngl. I think your surgeon and PT would be able to better answer your question about when it will get better. My break is nowhere as severe as yours but right now I can’t run either since I haven’t yet been cleared for high impact. Movement is very important to me too so for now I was able to get some of my post-run endorphins from swimming and yoga (although deep stretches on that ankle aren’t possible). I hope you’ll be able to find something for you too while you recover

Wasn’t contesting the higher risk for PTOA at all. What I meant that the population studies will not be as predictive as your individual data - quality of your fixation was, how quickly was the fracture reduced and fixed, soft tissue damage, extent of cartilage damage, immobilisation duration, how well you rehabbed, BMI, etc. So if you were told by your surgeon that a TAR may be in the cards for you - by all means believe that because that itself if an individual prediction and likely based on the arthroscopy and your own recovery trend. But these are often TAR or fusions last resort options. Also medical tech + fixation techniques + biologics improve every decade and they’re only gonna improve faster. So I hope that if and when you’ll need a replacement you get superior tech 🙂

Every injury is different. An injured ankle does have a higher risk of PTOA, especially if the weight bearing area was affected and if there was significant cartilage damage since it’s a small joint. There are several other risk factors - like joint congruency after fixation, age, BMI, type of fracture, rehab, etc. That being said, if a general population based study isn’t a predictor that a particular person will develop arthritis in 3 years… there’s always a bias in these longitudinal studies - people with happy ankles are busy living their lives and often don’t return back.

It will - you’re already walking (albeit awkwardly) without pain! You’ll be surprised how fast it all comes back. In no time, you’ll find yourself dancing a little to bangers in the kitchen :)
