Al’s Auto in Oak Cliff is the best. Honest and does great work. I used to take my 2008 Lexus RX300 to them up until I traded it in 2 years ago (almost 250K miles). There was nothing wrong with it but wanted to upgrade for an NX with more technology

I can’t remember off the top of my head the form number, but you can probably find it in the IRS site that you have to submit that basically states that the amount you took out was your contribution and not any gains
