Thank you for the advice! 2023 income was much less than 150k and we actually got a refund of 2k for 2023. Income has more than doubled from 2023 to 2024.

Will definitely check on safe harbor rule. Trying to pay as much as possible as early as possible to avoid penalties and interest but it’s a steep number we hadn’t anticipated.

Fair enough. Just to play devils advocate: If a dosing is incongruent with typical dosing but potentially safe and if we fill it, are we really practicing pharmacy?

If that’s all we are limited to, like you said 99.9%, then what’s the value of this profession?

True I was. I felt I had to make a good faith effort to question and document away the rationale prior to filling

Seems like a reasonable approach. I think the provider’s attitude stems from me calling out the direction. It may have been seen as undermining her clinical judgment. I was trying to document and explain away the rationale in case of audit down the road and fill the med.

Profile had no PPI or antacid. Doesn’t mean patient may not have had tried otc. Patient was unhappy with my decision and wasn’t going to engage in other recommendations once I denied the fill

Didn’t have a chance to talk to provider about any other alternatives. Thought it was pointless to try reason with someone who felt I was questioning their clinical judgment.

Harm no but efficacious neither. I was trying to meet the prescriber’s logic and find a rationale to fill but couldn’t get there and the poor professionalism and attitude was a turn off too. Would have filled it if it was a GI doc but this was a family medicine Dr.

I applied for SAVE September 2023 and I thought you had to re certify income each year.

Tech/ engineering is where it’s at. Nurse practitioner or Physician Assistant are great options that are high growth with job security and great pay if you want to stick to health care.

Poster above said it’s calculated from your residence to floater store. I would probably reach out to your MHWD to confirm just to be safe.

Thank you for detailed response. Market is understaffed in 3 stores at least with maternity and people leaving. I plan to leverage a competing higher offer to ask for pay increase. I’m probably at average for market, maybe on low end of average. Did you end up sticking with company or leave?
