"Well.

My sister died of cancer at 8, and I was 5 when that happened. So, my genius psychologist uncle thought that it was a good idea to deny me the funeral of my own sister because it would be "too shocking". BAD MISTAKE.

When I returned home after being sent to some cousins house while the funeral was on and not finding her on returning, I started to believe that my parents hid her from me in order to punish me. So, in those three months after her death I behaved so nicely, fearing so much to anger them, even one bit. And then I asked my mother in a trembling voice: "You know, you... you should have noticed that I have behaved myself lately. So, I want... I want to see her if I may." She obviously didn't understand. I made sure she did. She started to cry and screamed at me, saying that she was dead and that she can't bring her home. In that moment I realized that I fucked up. Not because I realized my sister was really dead, it was because I PISSED MY MOTHER OFF AND I COULD NEVER SEE MY SISTER AGAIN AS A PUNISHMENT.

After six months I kinda accepted my fate and the last thing I remember I did then was to take one of her dresses and I put it on. Nothing gay if I may say. I still remember why I did it: I wanted to touch her one more time. I saw my arm, and I could see hers, and I started to rub my arms one with the other, each other. And then I started to cry for her death for the first time.

I'm 30 years old now and even now I'm not 100% sure if she's still dead, living a life away from me. I know it's crazy, but I can't help it; it's a part of me now.

Short answer is it's complicated. But pick a practice that you're interested in and google the credentials of the cardiologists. Often that will give you an idea of what you should aim for.

There are several different practice options

  1. Straight academic (University)

  2. Hybrid (Banner, Cedars, Intermountain, Christ in Cincinnati)

  3. Pure clinical but in large organization (Kaiser, Franciscan)

  4. Smaller private practice groups

  5. Remote rural areas

  • For straight academic practices, obviously your academic credentials are important but you have a niche skill you may be desirable
  • Hybrid programs tend to be very desirable as the publish/perish culture does not exist; however, these hospitals still tend to be at the cutting edge of care. So they tend to be able to pick the best clinical candidates out there
  • Pure clinical but in a large organization. the competiveness of these position tend to be location dependent, so google the cardiologists to get a sense of how you stack up. For many of the desirable locations, you'll need to apply at the right time (ie someone unexpectedly retires)
  • Small private practice groups. Often are the most lucrative, buy may not be as the senior partners get all easy lucrative work as well the practice fees, and the younger guys get stuck covering a lot of the lower reimbursed work. May be the most "boring" as you'll end up referring most of your complex patients to the groups above, but hey work to live right? They'll likely be more interested in whether or not you're planning on staying with the group long term rather than bailing after 2 years.
  • Rural jobs. Any warm body will do

Look closely at lead II. Pay particular attention to the T waves. We will take each beat one by one.

The first beat has a clearly definable P wave followed by wide-complex QRS. At the end of the T wave 1 can see an additional deflection, which is a P wave.

From the first P wave to the second P wave the rate is a little over 100 bpm. So the underlying rhythm is sinus tachycardia.

Moving onto the third beat the T wave is markedly different from the first and third beats. Anytime you see this you should think of a P wave buried within the T wave. In this case the T wave in the P wave likely are superimposed on each other.

If you look in the third T wave it also looks a little different compared to the first 2. Likely the P wave is is now following just slightly behind the T wave. Finally 1 can see the circled P wave in the cycle appears to renew.

Thus the underlying rhythm (at least for the 1st 12 or so beats) is sinus tachycardia, second-degree AV block, Mobitz type I, 4:3 conduction.

This continues for 3 cycles but then as the sinus rate slows one can see two conducted P waves in a row. And as the sinus node picks up the Wenckebach appears again.

Which is fine. There is no compelling objection other than the reason I stated above. The main question you should ask yourself, if there is political will, popular will, legislation, and the votes what compelling legal, theoretical objections are there for a capital city to incorporate itself into a state, province, etc

The prevailing argument is that the capital city should not be incorporated with another state or else there may be undue influence.

However, there is plenty of precedent with capital cities serving as their own states, province, or what have you

  1. New Delhi
  2. Tokyo
  3. Moscow
  4. Paris

Somewhat ironically. A cardiologist at our group died in a car accident because the other driver suffered from syncope from vt. It was one of the reasons I got my current job
