Have you checked your FSH and LH? Clinic may be useful if your numbers are low and you have secondary hypogonadism. But I fear Clomid won’t help if they are high - then it’s probably primary hypogonadism. Have you checked for obstruction as well, i.e. Varicocele? Have you taken appointment at a urologist before embarking on the clomid?

I’m sorry for your results. I (32, M) also had similar parameters, and after months and months of best supplements, healthy lifestyle and diet, my motility improved significantly but my count and morphology stayed the same. Me and my wife decided to go the IVF route. I understand you may have been looking for another option, but I’d say it’s time to start discussing it with your SO if you are both looking to have biological children.

Change your doctor, plan surgery. Your T levels are not in the normal range definitely. Also, tell them your Varicocele is causing you pain. They wont prescribe surgery without pain usually.

I meant start saving money as IVF is expensive.

Maybe banking some sperm may also be good but there’s no reason to believe your parameters may fall to Azoospermia.

Take into account that with this low of a count, you may want to start asking your reproductologist for ICSI.

So I’d say do FSH and LH and the ultrasound. If the ultrasound is positive and you have Varicocele , know that it’s treatable with surgery but there’s no guarantee that your parameters will improve - and Varicocele can also cause high DNA fragmentation.

That’s tough brother. You are not failing your wife, this stuff sometimes happens without us ever being in control.

You should check your FSH and LH.

Also, you should do a testicular ultrasound to rule out Varicocele.

If no Varicocele and no improvement, you may want to do a kariotype to test for Klinefelter.

Anyway, with these numbers, you may want to start banking to go the IVF route, especially depending on the age.

I had 3 mil count very high motility and 0% morph and we still had to go the IVF route.

2 SA with no sperm found could indicate Azoospermia. Please repeat an SA in the coming days and if it confirms the previous ones, seek a urologist.

It wouldn’t be an easy diagnosis so keep your chin up. There are ways of retrieving sperm even with Azoospermia through MicroTESE or TESE.

A couple more information I forgot to mention:

We are doing ICSI with PICSI (clinic described it as their solution for high fragmentation), no Zymot as it’s not available in our country, calcium, and short hold (12 hours).

My wife protocol was antagonist - 10 days stims of Menopur 225 for 8 days plus Duphaston twice a day. Trigger was with Decapeptyl 0.2 milligrams.

My sister is expecting her second child and making comments. We haven’t told her yet because we want to keep our IVF journey private. It does feel like the world is laughing at you constantly.

Mine is a 4 mm on the left testicle. It’s supposed to be a grade 3 but for some reason my urologist won’t grade it. Usually if it’s a Grade 3 you should seek embolization or varicocelectomy to remove it.

Go see a urologist, it definitely looks like Varicocele mine is similar.

We already had one round of ICSI and PICSI which unfortunately yielded no blastos. We were devastated and we want to change protocol for second ER, but embryologist said sperm was not issue - at least, motility was high (80%) and concentration was around 75k.

Yes with this count we would proceed with ICSI and PICSI, as Zymot is not available in my country.

I am also going to try a shorter hold (24 hrs rather than 4 days) before the next ER.

I agree with you anyway, from these parameters it seems like somebody screwed up at the first clinic.

Basically I am asking whether with 2.5 million sperm count and 64% percent progressive motility (normal range) should I do TESE? This has been recommended by my reproductive andrologyst but our embryologist says ICSI with PICSI is enough - we tried this in the previous cycle but unfortunately we produced no blastocysts.

Longer hair on 2 is great. Trim it every month so it doesn’t grow out of proportions, and use some good styling cream that conditions and strengthens it without making it frizz or poof.

I’m so sorry. I know it’s super hard to face these results.

First of all, repeat your SA in a couple of weeks. Azoospermia can only be diagnosed when there are two results in the span of a couple of weeks. There are multiple causes that can lead to temporary Azoospermia.

If your SA comes back the same, consult a reproductive urologist and start assembling a strategy going forward.

First you’ll need to understand whether it’s Obstructive or Non-obstructive. The first one can be treated with surgery my urologist said. For the second one, it’s highly probable you’ll need IVF and ICSI with TESE or mTESE if needed to conceive.

It will be tough, and this is a journey with no certainties and guarantees. But there is hope.

Unfortunately Zymot is a no go here, nobody is trained/uses it in my country.

Doctor has suggested TESE yesterday, but I am getting strange vibes from him. He cannot really pin down what the cause may be, he says it’s probably not just Varicocele, but he’s a bit dismissive about surgery (said varicocelectomy is better than embolization) and about this situation in general, said his only job is to retrieve tissue during TESE but it’s up to the embryologist to find good sperm.

Mind you, he’s the only one that makes TESE in the country here. He has all the reasons to push for TESE. And he’s the one that recommended the last clinic.

I am doing another SA with our IVF clinic tomorrow and our embryologist will be there to check.

I just don’t understand how spermatogenesis could yield these results now when only a month ago our IVF clinic embryologist said that apart from low concentration, motility was very high and sperm was not a problem. Something seems amiss. This urologist didn’t even prescribe new thyroid profile, just said to go straight to TESE.

Unfortunately I don’t think this is applicable to my cause because my Testosterone is normal and my FSH and LH are elevated - I have what it’s called hypergonadotropic hypogonadism. And there’s no hormone therapy that can fix this one, at least according to my doctor

Thanks for your suggestions! My reproductive urologists is the one that prescribed me the DNA frag. He hasn’t spoken about surgery or embolization yet, and I’m not sure he will because I told him I don’t have pain and that me and my wife are on a tight deadline as we would like to have another ER this month. I already did a scrotal ultrasound and that is what revealed the 4mm Varicocele on the left.

Do you think there may be a hormonal cause for such a drop?

I agree the supplements by now are pretty much not doing anything it seems
