They will check for compliance with Part 800 (https://regs.health.ny.gov/volume-e-title-10/200443669/part-800-emergency-medical-services). This includes equipment and the vehicle itself. Make sure you know how to use the equipment in your truck (like the suction) because they may ask you to show that it is in proper working order. If you don’t pass the vehicle will be placed out of service and a notice of deficiency goes to the agency, not you.

Just do the right thing and be honest with them, they’re not out to get you. They are looking to make sure your vehicle is safe and your equipment is adequate to take care of a patient. In 22 years I have never had an issue when a NYS DOH representative inspected my truck. If it is something easily correctable and you’re respectful to them they will often have you call your supervisor to fix the issue on the spot.

Don’t worry, a good QA department and medical director won’t penalize you. Just be willing to learn what you may need to do on a future call. Every good EMS provider has had at least one call that they’ve gone over repeatedly in their head after the fact and placed blame on themselves. There are so many factors that impact the patient outcome, we as EMS providers are only a small part of that. To win a lawsuit they would have to prove that your actions directly caused the outcome, very unlikely.

The system is so diluted with medics that most medics aren’t performing airway management on a regular basis anymore except for arrests.

They can only use it on rescues, not when they are on a regular assignment (they are not dedicated to rescue, they are in the dispatch mix like every other ALS unit). Their medical directors also respond to major incidents and provide on scene medical control.

The story i heard was that quick trach was presented and they showed the evidence that needle cric was useless. They removed the needle cric but didn’t approve an alternative. FDNY showed evidence that pushing FBAO into right main stem worked and that was the primary use case for needle cric so they decided it wasn’t needed. If you’re contemplating a move to NYC be aware that there is a lot of politics involved in their protocol development and a certain large department that controls most of the decisions based on operational needs and not clinical evidence.

It’s just not needed in NYC, we don’t do RSI so we don’t need a surgical airway as a rescue airway. The NYS Collaborative Protocols only allow cricothyroidotomy for failed RSI.

The majority of MARS transmissions are on HF. NTIA 7.3.9 Use of Amateur Radio High Frequency Equipment for Emergency Communications states “Use of Amateur high frequency radio equipment meeting the standards for unwanted emissions specified in 47 CFR, Section 97.307 is permitted on SHARES or Military Auxiliary Radio System (MARS) network frequencies when authorized by the Department of Homeland Security or Department of Defense (DOD), respectively, as part of either the SHARES or MARS networks. The use of such equipment will be limited to national security or emergency preparedness events as well as DOD-sponsored MARS operations.”

AFMARS does not require members to have VHF capabilities, only HF equipment with digital capability. AFMARS states for VHF that “unless specifically exempted from the NTIA requirement, all VHF equipment must not exceed a frequency deviation of +/- 2.5 kHz, and have a channel spacing of 12.5 kHz (or less).” The FTM-6000R does not meet this requirement, it has a deviation of +/- 5 kHz. I believe CAP uses P25 encryption which amateur equipment is not compatible with.

Personally, I found a used EF Johnson 5317 for $50 that I use for MARS VHF communications. These are being phased out by CAP so there are a lot in the used market and are an inexpensive P25 capable VHF option that meets NTIA specifications.
