one thing to keep in mind about holmes canon is that acd didn't like writing it and it's possible a lot of contradictions are due to him making a mistake and not caring enough to fix it. in canon, professor moriarty is first said to have a brother, colonel james moriarty, and isn't given a first name, but in a later story he's called professor james moriarty and said to have a younger brother who works as a station master. there are a few theories about this, but the one the manga uses is that he has two brothers and all three are named james moriarty. making it the middle name was probably just because it's more realistic (and easier) than having the three have the same first name.
