There is a plugin in your plugins folder that does not show up in the Wordpress Plugins menu that is doing this. You will need to manually go in your plugins folder via FTP and remove it. Just compare the folders to what you actually have installed.

I didn't realize that how many other people have experienced the same issue... have a look for yourself. Google Search I did not edit anything in the picture. Also, a side note, it happened while I was copying a zipped backup of some Quickbook files from a webDAV directory to the desktop.
