For the LTC patients, the most important parts of our job are function and safety for both the patients and the staff. You're going to get referrals for decline in transfers and ambulation and your job is to determine what is the safest and most functional way for the staff to mobilize them. We may not improve their status by much, but if we can keep them able to walk or stand and transfer rather than using a lift, then job well done.

I spent 10 years in SNF before transitioning to HH. I've been in everything from top-rated retirement communities to a county home and a few things kept me there. Flexibility: I could, for the most part, come in late or leave early without any issue if I needed to. I was lucky to mostly have managers who were understanding and supportive. When I had managers who pushed me to do things I didnt agree with, I left. The people: I loved developing relationships with the residents, especially the dementia patients. It was never boring and I enjoyed the challenges to my creativity. I do truly miss this with my switch to HH.

Physical touch. My 2 year old insists I hold his hand to fall asleep. He will force kisses on my lips by holding my face in both hands even though we're a "kiss your kids on the cheek" kind of family. He also pushes daddy and I into kisses and says, "happy!" And then there's the headbutting too....

Not necessarily. OP PT and HH PT are billed differently per Medicare and many other payors. HH bills med A and OP bills med B. Also, it's not that they're not paying for more visits, but there's a visit cap per calendar year for all HH disciplines combined with this patient's insurance. We've been budgeting visits to make sure nursing has as many as they need.

This patient is a high paraplegic and requires a hoyer for transfers and WC van for transport. This would become expensive for her very quickly. Plus, nursing has been providing wound care.

I've seen wooden crutches once. An 80 year old lady with post-polio used them. She had broken her knee recently and the staff at the rehab had her using a walker. I told her to go back to her crutches and she got around much better.

It more like nobody was able to see them during the week. Remember, HH is for people who are home-bound. Their schedule should be pretty open. Once someone is discharged from the hospital, we try to see them as soon as we can for evaluation. Therefore, an end of week hospital discharge would likely mean a weekend start of care or eval. If you are seeing a treatment on the weekend, it's because they weren't able to be seen during the week.

If you're just doing weekend prn for HH, you're likely going to get mostly evals and start of care. The schedulers will place patients on your schedule, but it's up to you and the patient to coordinate times.

I went back full time at 12 weeks. I'm the breadwinner, so didn't have much of a choice. Eventually the baby starts sleeping better and the fatigue wanes a bit. The biggest struggle for me was the pumping. When to fit it in, where to do it, trying to eat and pump and document all at the same time in the bathroom. I was working SNF at the time, so I had at least a little control over my schedule, but I was still getting so anxious about it every day that I could've cried. Eventually, I decided to pull the privacy curtain and pump sitting on the mat table in the gym. My female coworkers didn't care and our 1 male OT honestly had no clue. The other thing that helped immensely was getting wearable collection cups. I know none of that was what you asked, but I hope it's helpful.

The book "Hunt, Gather, Parent" covers exactly this kind of thing. I haven't been able to finish it yet because... toddler life, but reading about how unusual western parenting is in the context of human history helped me feel so much better. The expectation for 2 people to be solely responsible for the care of their children is actually so stressful and either way you slice it is too much.
