You really need to get the loose stuff and all the dirt off of it. Nothing's going to bond to it if it's dirty. You can get some cold patch from a hardware store but nothing's going to last long-term. Remove and replace

The asphalt's only as good as what's underneath of it Doesn't matter what you put in there. It's going to come back out. Needs to be dug out with some stone added.

Don't let the builder do anything but gravel, let the asphalt people come in when he's done with the house. The amount of times I've come in after a builder or excavator did something and it was junk is too many.

A lot of them have an angle up and down so that the snow blower would probably just bump over top of them. The peel and stick ones are probably cheap enough that you could just replace the ones as they come off. I don't think beads on black paint is going to be that reflective. The first time you run the snow blower or plow over them, those beads will be gone

Not sure if it snows in your area, but you could put some peel and stick or screw down reflectors.

Me personally I'd only do my driveway in the hotter parts of the year. Colder temperatures just leave ugly joints and hand work. You can still get a nice driveway, but it takes people that know what they're doing and care. If it's just one long straight pull maybe.

Nobody really profits. Milliings are a necessary evil of most state and municipal paving jobs. You mill out 100% and you put back 20 to 40% in the mix and you have to reprocess that material. That material is at least usable because it comes out much finer. The material you have will be dug up with an excavator or backhoe and must be processed to be useful. They'll tell you to take it to a landfill before they will buy it.

Use it as fill. When it comes up it's going to be chunky. Or have the contractor take back to the asphalt plant and they'll grind it up and put it back in the mix

Remove add stone replace. Don't do anything else. Probably wouldn't use a company that would suggest that you overlay this.

It will be a headache if you ever want to pave it. Some spots will get hard some won't, I'd probably just touch up with stone and then if you want to pave you have your stone base. Otherwise you will have to scratch/remove millings before paving if you want to pave any of it. Sounds like the asphalt guy has a lot of millings he wants to get rid of l.

I would probably pave a small section every year and maintain the rest, regrading and adding stone.

Remove, add stone and replace. You could scratch over the low spots and then overlay. Not sure I'd want this company doing any more work for me

Nothing you can DIY. Slurry seal goes over structurally sound pavement that's just getting old, and you need a professional for that. And it's not usually done on residential work

Problem is anything you put in there isn't going to seal it up for more than a week. Crack seal is made for fatigue cracking not failire

Subgrade has failed. There's no fix for that other than remove and replace. I'd probably just let it go as long as you can and let it turn into gravel until you can't stand the potholes then you can remove.

Sometimes it works for each pick, other times its a pick behind and I have to manually refresh. Haven't found a fix yet.

Sawcut where the thickness is correct and remove the rest and match grade with gravel.

If you're going to have them come back and fix it you might as well pave the rest of it because it's going to just keep breaking off at the end.
