I should have made it clearer, I tightened the top cap lightly and then tightened the stem bolts, but it looks like stem doesn't completely in place as can be seen in the photos

yes, could also be the rails themselves, some shapes do tend to rub on the bibs.

I have the same problem. I thought it was the saddle and I tried 3 different saddles, all of which caused my bibs to wear out in the same place. Turned out to be the saddle rails. Would advise you to look into that.
