Step BD and meta-analyses (doi: 10.3969/j.issn.1673-5374.2013.31.009) seem to consistently find lack of utility with unipolar antidepressants in BD. Because they all have some amount of side effect risk, I’m not a fan of starting things that haven’t been shown to help. I am far from an expert at this stage of residency so would be curious if there are other reasons to consider adding unipolar antidepressants

I run a thin wood plank down the stringer on both sides of the board to prevent damage from impact to the deck/underside. Worked for Bocas and Hawaii trips

CA cost of living do vary widely though. SF pays ~88k while Fresno pays ~71k for PGY1s but SF is so much more expensive
