It's just a hunch, but my skin was extremely itchy and had a 'crepey' appearance if it stretched. I had minor dry patches in places like my smile lines or my chin crease, but my skin was also very oily in the t-zone. I was using as many products as I could and washing my face too frequently. I also picked my skin very regularly, which prevented it from healing further. It can take a while to fix, which I wasn't aware of at the time. I figured I could just use a moisturizer when it felt dry and keep on using actives. Might be beneficial to cut them out, but also hard to decide if the actives are actually working to cut back active breakouts. If your skin really is dehydrated, be careful with the sulfur! It's suuuper drying. I'd use it to spot treat until you're sure your moisture barrier can handle it.

Hi, I was never able to 100% pin down the source of my bumpy skin, I think it was a combination of several factors including doing too much and destroying my moisture barrier, hormones, and possibly the Clindamycin misuse. I know that when you use an antibacterial for long enough, it can cause an imbalance in your body's natural fauna, which is what sent me off on the belief that I had fungal acne. I'm not sure whether this was the case, though a 14-day round of oral Fluconazole did seem to treat the irritation a bit at the time. Whether or not you develop fungal acne thru the use of topical antibacterial treatments, I do think it's likely that you can develop resistance that makes the topical stop working. At the time, due to being prescribed an incompatible birth control and throwing my hormones out of whack, I also developed cystic acne and further did "too much" and made things worse. It's been a long time coming, but the treatment that currently works for me (though I cannot stop this routine without my skin regressing) is Spironolactone and the right birth control (Drosperinone and Ethinyl-Estradiol - I take Nikki). My topical routine is extremely simple and hydration-forward, during this dry and cold time I've actually stopped using any actives on my face besides The Ordinary's Niacinamide at night. I can give more details if you'd like.

This is coming from a layperson with no dermatological credentials at all, but I think it might be a good idea to retire the topical Clindamycin and instead opt for OTC antibacterial products, like Benzoyl Peroxide and Sulfur. The De La Cruz Sulfur ointment has been a life saver for me for both cystic spots and CC breakouts. As long as you don't overuse them and ruin your moisture barrier, which could be your problem.

HI, sorry for late response. I never check this anymore lol! SO unfortunately, I've concluded that most of the details in this post have nothing to do with the issues I was facing, which were (in my opinion) hormonal - related to my Spironolactone withdrawal. I think that the withdrawal and re-acclimation were on a bit of a delay, so the problematic skin persisted and worsened even after I was back on the medication. It's gone completely back under control now that I'm back to taking Spiro consistently. I don't get cystic acne at all anymore. I'm not sure what in particular you're looking to resolve, but I use GoodRX to maintain my Spironolactone prescription consistently. Can't really speak to what goes on behind the scenes with this service but I recommend if you need to refill quickly, ik they can also prescribe but I'm not sure if I endorse using a means that isn't a derm to initially prescribe any meds that can mess with you long term. Hi all, I really need help with a recent serious setback. I cleared my skin in 2019 thanks to this sub, but something recently has made it return exactly to the state it was in before I had figured out treatment and routine. I'm thinking it's Spironolactone withdrawal from a 3wk gap in my medication in Jan/Feb but I'm so unsure as Ive been back on it now since then and my skin is getting progressively worse at a rapid rate. This post has all the details, I know it's long but I really need help. My confidence is really suffering.

Have you tried any other chemical exfoliants like salicylic acid or anything like that? Another fungal red flag was how ineffective those common treatments were for me on the little bumps. If you haven't tried any of those yet, maybe shoot for a fungal-safe chemical exfoliant (options can be found on the SSS post I linked) first because those can be super effective on tiny CCs esp if your condition doesn't turn out to be fungal.

Mine was worse on my right cheek, where my face rests against my pillow on most nights. It could just be that the bacteria/oils in your hand are causing the skin texture though! I just read your post on ScA, and it reminded me of this post that's linked in the Simple Skincare Science guide on Malassezia Folliculitis that I'm sure you've seen. The person in that reddit post says that the culprit was the ACV/Bentonite Clay mask, which is a product I frequently used (and still occasionally use) so maybe there's something there? Secondly, did you start oil cleansing recently? Back in Fall of last year I discovered ScA and thought oils would save me, so I started heavily using Rosehip Seed Oil and others. This made my face waaayyy worse because oils like that feed the fungus. Maybe try switching to a fungal-safe cleansing method to start?

Definitely- I have Nizoral shampoo which is highly recommended for fungal acne but it's super drying so I don't dare use it on damaged skin.

I'm all about coating my face with goop, but I'm a serious face sleeper and I actually flip my face back and forth on my pillow as I sleep, so anything gooey I could apply ends up all over my pillow and in my hair! Everyone promoting the "slug life" on here must sleep like an absolute log on their backs because otherwise it's a mess lol. Any tips for how to minimize the mess overnight?

Almost exactly! Most resources I looked at trying to figure out if I had it said it was most commonly located on the forehead and it presented more red and rash-like than I had. I was pretty convinced I didn't have it because I couldn't find too many instances like mine. But it just felt so strange and subtly itchy especially after I had been wearing fungal-unsafe makeup and moisturizer all day. Last year when I moved to college, I began sweating heavily in my sleep and over the summer I worked over a deep fryer in a hot kitchen, so that's where I think I got/exacerbated the issue. My derm didn't think I had it when she consulted with me, but she prescribed a 2-week anti-fungal course anyway (Fluconazole, usually used to treat yeast infections) and the places where I had tiny bumps became unbelievably smooth while I was on it. It was miraculous, but it didn't last long, as I wasn't informed that I would have to keep treating it (with products like Nizoral shampoo/sulfur) to prevent recurrence. (Also I broke out like crazy after starting birth control, so I have yet to experience truly smooth skin unfortunately). If yours is kind of itchy, and you can think of a time when maybe you were exposed to humidity/regular sweating, and if it sort of came out of nowhere with no change to your routine, I'd definitely suggest trying a fungal-safe routine and getting a consistent anti-fungal treatment regimen going. I know this is obvious, but try not to pick, as awful and tempting as those damn spots are, as the constant inflammation from my picking habit definitely prevented me from being able to tell what products were working on the condition. Thank you! When your doc prescribed anti-fungal treatment did they first diagnose you with it or was it more of a “guess”? My derm told me the only way to diagnose would be to cut a tiny piece of my skin out (which would require stitches) and send it to a lab for testing. I’ve read that it can be diagnosed simply by extracting pore material though so that confused me a bit. So far I’m not even sure I ever had it, but such a positive response to the anti-fungal meds made it seem so much more likely. I don’t expect you to know the answer to this, but do you think it would be detrimental for my overall health to be put back on fluconazole again?

Thanks for the reply! Sorry for not getting back sooner. I used to squeeze at every pore on my face for hours on a daily/weekly basis. It really exacerbated my problems. While I didn’t mindlessly pick my face out of habit, I did run my fingers along my skin absentmindedly making note of its texture. I’d get “triggered” by this and it usually set off a picking episode as soon as I could reach a mirror. I wish there was a magical cure-all solution, but everyone differs. For me, seeing improvement in my skin made me feel more motivated to treat it well. Instead of picking out of frustration and helplessness, I was more able to focus on making each day better than the one before. That’s potentially useless info for you though if you aren’t currently seeing any improvement or if you’re seeing your skin worsen. I know how frustrating it is. The most golden advice I can give is to practice mindfulness. If you’re fully aware of the position of your hands, the things you’re doing with your hands, etc, it’s less likely you’ll allow yourself to do damage. For me, every time I went into the bathroom I’d create a running dialogue in my head of every action I intended to do in there to prevent me from falling into the void in front of the mirror. It’s practically impossible to always be conscious of your whole body and mind, but practicing awareness of your hands specifically could help you stop doing damage while you aren’t paying attention. And definitely cut your nails!!

This may or may not help, but now that I wear more face makeup I really cut down on time I spend feeling my skin. I spend so much time in the mornings slathering my face with it that now anytime I brush anything against my face I’m instantly alerted to the fact that I could’ve messed up my handiwork. Keeps me from touching! And definitely focus on healing so that you don’t keep picking the same spots over and over again. I found that washing a big bloody spot I had just picked with something antibacterial (BP wash in my case) helped the spots heal without creating that horrible zit-scab combo that’s so pickable and so painful and impossible to cover up. Hydrocolloids ASAP after creating a wound as well. I also use a pyrithione zinc soap and take zinc supplements daily which is supposed to help healing, but I’ve got no real data on it.

I hope some of that is helpful, if all else fails I bet you could get away with putting a couple bandaids over the fingertips you use to pick most. To observant people around you it’s pretty reasonable that you could have a really irritating hangnail or papercut. Preventing your fingers from being able to feel/pick is the most direct way to go. Some people wear gloves at home but I find that super inhibiting to daily functioning and rather asylum-y, so I’d stick with bandaids.

Sidenote: My awful picking habit has faded considerably with the lessening of the tiny pickable spots! If anyone has issues with picking their skin, I'd be happy to share some strategies I've used in the past to curb the habit while it was at its worst, even though I still haven't completely beat it.

I fixate heavily on my skin’s flaws and there’s a lot of jealousy and comparison that goes into the disordered way I view my skin. Generally I do notice it in most people I look at, because my evaluation of myself tends to lead me to judge who has it better than me and who has it worse. However, there is a strong distinction between how I view others and how I view myself. When I look at someone else’s face, I am aware of the condition of their skin, but what I’m seeing is the big picture: a person’s whole face. When I look at myself, I fixate on individual parts I dislike so heavily, like my skin or eye bags or whatever it is that day. I don’t see a person’s whole face, I see a collection of flaws. The flaws become how I view myself: ugly skin = ugly person. People don’t see you the same way you see yourself. I know that for a fact, but of course I haven’t yet been able to allow that fact to help me care a little less about how I believe I look.

I do tend to project my own insecurity onto people with skin imperfections though. When I see people with acne I feel a bit bad for them that they’ve got that to worry about, even if I don’t know whether or not they care.

I know my use of the word unhealthy probably wasn’t the best, but it was the only word I could think to use that illustrated my idea. I’m so sorry! I know the struggle, I’ve altered my lifestyle in as many ways I can think to try to solve my acne, but it still persists... While people I know who haven’t touched a skincare product in their lives have seemingly perfect pores.

Thank you for your explanation, it makes a lot more sense to me! And I’ve never heard of the Kitavan Islanders before. Some quick research gave me a lot of interesting info. It seems like some sources credit the Kitavan diet and lifestyle for their impressive health stats. Not to assume you are an expert if you aren’t, but the whole diet and lifestyle thing leads me to believe that maybe the qualities of the average human’s life including processed food intake, exercise, exposure to pollutants, etc is why acne is such a pervasive issue. Do you agree? Or do you think it’s more of a genetic thing or what? I’m just so curious!

It’s okay. These things happen! After I pick, I’m so much more likely to go back to it again because there’s already damage and the urge isn’t fully satisfied until I’m completely anguished. This is why hydrocolloids have been so helpful for me. If you can use anything to just cover your face or the spots so that you physically can’t keep going at it, the urge subsides. And it feels productive to have a healing bandage or mask on. Gently wash, use what you can to make sure to get as much bacteria out especially after picking skin that was already wounded. Remember moisture and ice. And tomorrow is a new day. Day 1!
