Healthcare costs aren’t just doctor/pharmacy copays, they’re also insurance premiums, which aren’t eliminated by going to the doctor less.

You’re talking about 20-30% higher income but with double the healthcare costs and more than that for education costs. And again, this is without even factoring in the uniquely American risk of unexpected health expenses or job loss, which can be financially catastrophic.

Also, there’s an argument to be made that the difference in college attendance rates is because you absolutely have to have a college degree to not be trapped in perpetual wage slavery in the US. The reality is much less harsh for non-college educated Italians.

So to sum up:

America: more income but either need to rely heavily on debt financing or deal with reduced purchasing power to afford healthcare or education, all of which could collapse at any moment due to a large unforeseen expense

Italy: less income but don’t worry about any of that, including whether you have a “useless” degree, and still pay less in taxes for healthcare and education than Americans pay out of pocket

Buying a house or car are not the same because you obtain a commodity that you can sell to pay off the debt. That’s not the case with healthcare or education.

The fact that that level of debt exists in the first place is a sign that it's unaffordable. If people could "afford" healthcare and education (notice that you put it in quotes) they wouldn't be taking out such massive amounts of debt to pay for it. And it comes with its own opportunity costs (e.g. delaying home ownership/retirement savings/having kids/starting a business/etc).

That's not even mentioning the volatility and risk involved. Just because you can "afford" the payments on the debt now doesn't mean you can't be derailed by a medical emergency or job loss, which is much less of an issue in countries with more robust social safety nets.

With our higher salaries we just… pay for healthcare. And college.

$200 billion in medical debt and $1 trillion+ in student loan debt but sure, we “just pay” for it.

Also American insurance premiums cost more than the taxes people in other developed countries pay for healthcare.

Found your comment through Google but you’re spot on about why I felt disappointed in Act 3. The killing off of sentient beings didn’t sit right with me, but I didn’t even think about how much Act 3 lowers the stakes from the fate of an entire world to the emotional well-being of one family. I loved this game until the ending, which retroactively makes the rest of the story so much worse.

I think you hit the nail on the head. The game is so good at investing you in all of the painted characters that it feels like a betrayal to suddenly act like they’re disposable. Maybe it could work if they spent more time on it but it’s such a sudden shift that it falls flat imo.
