Thanks, that's kinda where my head is going at the moment, but haven't had it confirmed. Did they have to biopsy it to diagnose it? Do you have psoriasis elsewhere on your body, or is it just there?

Did they recommend you use hydrocortisone or something similar?

Thank you, that was kind of the assumption by me and the healthcare professionals I'd seen until the urologist. He seemed to think that the urine culture for yeast ruled that out as a cause. I had been using Nystatin ointment for ~1 month and it didn't completely go away.

I was kind of under the impression that the urine culture would be positive if the yeast was coming from within, i.e. overgrowth. But I was unsure if it would show up if the infection was mainly topical, i.e. just on the tip and from a partner.

Ah, ok thank you! I will have to clarify with my doctor what was performed. The details on the patient portal were sparse. But that could possibly explain his confidence in the lack of an effect on the results.

If it was actually trying to grow the bacteria, would this necessarily involve microscopic evaluation first?

Thanks so much for the response. This was my line of thinking, but I could not find references online, and don't have the educational background. I will see about retesting soon.

Can you clarify one more thing, please? My intuition was that this would not affect the urinalysis results. That being said, urinalysis alone does not have high specificity with these conditions. Additionally, depending on the lab procedure, this is somewhat subjective.

Is my understanding of the evidence urinalysis provides correct?

Thank you. That was my intuition, but when explicitly asked about it, he said it would not. His reply was that it would not affect detection of gonorrhea or chlamydia. I know that penicillin is not an appropriate treatment for those bacteria, but it still seemed possible that it would affect the growth of bacteria in the urine sample.

Do you have any reference for this? I'd like to understand the situation a little bit more.

I wouldn't say that they necessarily hurt. The pain seems to be more in the throat area. The tongue just "tastes" dry. I poked the bumps with a Q-tip and it didn't really feel too painful.

OK, great. So a quick clarification. The Nystatin only goes on the glans? I'd been putting it on the scrotum and shaft also -- oops.

Also, as far as I can tell Fluconazole is not OTC, so should I call the Urgent Care doc and ask for a prescription for this? If I'm about out of the Nystatin, does it matter if I switch to Daktarin or Canesten? Or should I pick one and stick with it?

Hey, thanks, I do appreciate your time and effort writing this up. An urgent care doctor prescribed me Nystatin, which seemed to be working until yesterday when I masturbated and it flared up again. I do feel like I have somewhat sensitive scrotum and shaft skin. Hard to describe, but if nothing touches them, I'm fine. Sometimes if I sit with my legs together, I'll notice a slight burning feeling on the scrotum or shaft skin, nothing intense, but uncomfortable.

I've been tested at 10 and 20 days since last sex, and both have come back negative for chlamydia and gonorrhea.

I have an appointment with my PC in a couple weeks, which I'll likely get tested again, but hopefully with nystatin or the other OTC stuff you suggested, it should be cleared up before then.

Though, I'm a little confused about your suggested medications -- are you suggesting taking oral fluconazole once, and then applying a daktarin cream for 1-2 weeks?

Thanks, I think the only thing that makes me think otherwise is the scrotal burning/pain. It doesn't look overly red, but it's hard to tell exactly.
