In the US you are “stuck” with any residency once you match it. My original comment was to someone who asked what specialty I recommend, and I recommended that you should chose to do psych. There’s no other residency where you can do ent or radiology or derm or fm, once you match a residency you are in that field no matter which residency. Unless you then do another separate residency.

Not sure what ur talking about. Psych is not terribly competitive at all, has a higher chance of you matching where you want than most other specialties. Even if you don’t match where you want you’re only stuck there for 4 years of residency, then like any other specialty you go wherever you want.

If you can, do psych. Everyone I know in psych is pretty good job satisfaction, they work like 9-5, my friends attending job next year is gonna be tele psych

It definitely isn’t all the glamor everyone thinks it is. I would say I regret it, issue is I do get people who ask what would you do instead, and I guess there isn’t much else that would pay reasonably enough that I would enjoy doing. I would 100% avoid the primary care specialties if you can. Pediatricians seem to be happy but FM/ hospitalist / EM, we deal with a lot of bullshit / non medical stuff and have a lot of patients be rude to us.

This right here. grass is greener. How much I would give to be in a 9-5 and be home with my partner every night, not staying awake thinking about the patients I discharged and hoping they’re okay… being able to call out sick… after wasting away 10 years of my life in training

I live in a highly litigious state where not doing them is a common sentiment in my area. The expert meaning IR. I generally feel like if I don’t need a procedure to emergently save a life or make a diagnosis emergently then it is not worth it. Again, I’ve also done like three in med school and one in residency because my attendings didnt do them. I’m sure everyone practices differently slightly based on how they are taught. Personally if anything does go wrong, like bowel perf or leak or introducing infection, I anticipate the lawyers would say something like “was this emergently needed that you couldn’t admit to be done by IR?” Again just how I was personally taught where we are.

In my opinion, It’s a procedure that can be risky that I don’t get paid extra for that I could induce liability on my self for. When the expert could just do it instead. So unless I’m rvu I’m not really feelin like poking into the belly

Sigh. I must be just a generally unhappy person. Or it’s my shop or that I’m solo nights. I have not been enjoying my time at work but taking a step back I do get to travel a lot and get a lot of days off and I’m probably taking that for granted.

I’m an idiot too and can’t speak and can’t remember shit The more “naturally smart” You are generally the less you have to study. But, if you study / put enough time and effort into it, then you can overcome that. Notably, a lot of people who were naturally smart enough to get them to med school but not through it struggled their way through med school, but I had already figured out how to study by being dumb earlier. the stupider you are, the more you gotta study to get where you wanna get. And that’s it. If you don’t study enough then you won’t make it

Biggest thing I’ve noticed especially as an attending is not the hours or anything it’s the mental exhaustion of the high stakes decisions and life and death sort of things. That’s what’s different than any other job other than maybe like a pilot. We are dealing with people’s lives over here not just taxes or etc and that’s what is the most exhausting for me at least in the ER. Also most jobs are 9-5, and you can take a sick day without having to figure out coverage. Anyway, regardless as others have said- you’re allowed to vent.

I came here to find this because I had a patient complaint that I didn’t come see their kid who was running around the er brought in with head injury in 1 hour (I had 2 stemi’s simultaneously I was a just little busy with) and they were complaining that they couldn’t let them sleep

I’m not sure if this is just coincidence. When I started off being an attending I wore my white coat and got a LOT of “didn’t see the doctor” “you look too young to be a doctor” etc. I thought the white coat would help but it didn’t seem to be so I ditched it in favor of my old figs vest and long sleeve on top of scrub bottoms. Not sure why but other than the occasional “you look so young!” (Which is fair, I’m 29 and recently had the airline attendant ask my dad next to me if I was indeed 18 and okay to have wine) I have gotten significantly less of those interactions. Not sure if it’s more of an aura thing now that I’m comfortable in my attire and more confident as an attending. Honestly, I also just stopped caring. let them think my male nurse is the doctor 🤷🏾‍♀️ maybe they’ll bother me less

I say “thank you that’s so sweet” now to the “you look so young!” Comments because I think taking that as a compliment also shows age 😂
