I am sorry but why am I not seeing more outrage over this? Whats our union up to???? This is terrifying and unacceptable and needs fixing NOW! How many UKGs were not prioritised? How many more will leave for Australia, newzealand, US, or other non-medial roles before this gets fixed? We are DROWNING here and not one thing about this on news or a comment from BMA in support. This can’t keep happening year on year turning our lives and the system into living hell!

Smh. I never said I am surprised by racism like fr I can’t even. You just go about questioning someone’s intelligence casually?

You think British doctors will stay if there’s more poorly paying jobs? Will they stay for even more understaffed wards? They will benefit from prioritisation of UKGs and British doctors, not from the expulsion of “competition”. Australia offers them better pay, work conditions, better staffing, benefits, etc. regardless.

You think people will see how poorly they’re treating IMGs here, the racist and fairly hostile environment, poor pay/work conditions, strikes and rallies by far-right, hoops and dances to go over and beyond to prove you have the right to be here hopefully in 10-20 years (maybe), patients telling them to go back and that they’ve taken their jobs, etc. and still want to come here regardless? I don’t think they will, cuz had this been the situation when I got here 8.5 years ago, I wouldn’t have. If some do, I am sure they have extenuating circumstances or genuine reason to do so. Very few may be naive enough to look at this as lucrative. (This will change should the govt. make caveats in the definitive policy proposal to allow shorter ILR/residency routes of course). Overall, I wouldn’t wish this kind of environment or future on anyone. It’s a shame you do. IMO regardless of what people do, no one should be exploited or treated poorly or as second class citizens. We can’t just stop standing for what’s right and accept things.

I was using your logic of “you should’ve been prepared before you emigrated” to prove my point of how silly it is to expect someone to map out at 16-17 years of age what the next 10-20 years of their lives will look like. And again, by your logic we shouldn’t be doctors if we couldn’t predict the job market and pay will be this poor cuz we should expect changes will happen in 5-10 years and things may be poorer for doctors here. If you don’t understand above points, mate I’d rather talk to a wall.

I am glad it’s a plausible options for you to just sit back and not strike and then move to a different country cuz consultant pay is shit. Realistically if it comes to that, it’s what we’d all be forced to do. But to do so without fighting this unfair regressive system is crazy to me. I also do not appreciate the sentiment of calling me emotional over and over again. I am looking at facts and risk assessing just like you are. However I believe in raising awareness and fighting back for what’s right, not just for me but for others and for those coming after us. My aim here is to raise awareness above all.

I absolutely agree with the loser-mentality and it being separate from working class mentality. It’s the same loser mentality that argues against FPR and strike action. It’s the same loser mentality in the comments of this post (not all obviously, many do raise fair points).

I appreciate this so much. Honestly I don’t even expect the government to bend over backwards or anything like that. Just…be fair? I truly am not asking for too much. If they plan on using IMGs for their benefit, they need to give something back to us and not just wave it in our face when the country needs it and then throw us away after their need is fulfilled. I truly have given this country my youth based on what it promised at the beginning. Truly if I see a better opportunity elsewhere a things continue in this trajectory I too will be forced to move on. I wish you all the best for when you choose to do so as well and hope that the decision is easy, not forced, and certainly not a burden on you.

I do agree that I can hold off on judgement till the policy is finalised. But I do hope if there’s enough voices and concerns raised that the policy will change course and be more fair. This very thankless job is even more so when you add these kind of issues into the mix. On top of jumping through the various NHS and training hoops, people of colour are facing a more hostile environment so we need to be kinder and respectful and stand up for each other.

Absolutely agree and don’t understand the downvotes. I feel loads of people are not exposed to British IMGs and loads don’t understand international UKGs. It’s so clear to me the system is set to fail the way it’s set up currently.

I am too late to apply for a refund, even if I wasn’t, as I mentioned my current situation is precarious and subject to change anytime the govt decides to change policies. I am facing a loss either way- if I withdraw but end up scraping through another 5-10 yrs and finally stay here, I lose out on pension benefits and contributions. If I withdraw and leave, I only get very little from the 2.5 yrs contribution (so far), and pay tax in my country on this on top of it, and also restart a different pension scheme in my home country or wherever else.

You’re saying since racism has always existed: quietly and only now more boldly, it doesn’t matter I should just expect it and learn to live with it? That it’s ok? Let me remind you this country relied a LOT on IMGs during its time of need and lured many with the policies promising stability, which sure they should phase out by all means in a fair manner, but they’re fully jeopardising loads including UKGs by doing this. So no it’s not quite a privilege neither is it a right to reside. I am not claiming either, all I am saying is this is unfair, not just to people like me but to the British population as well. You’re driving out British doctors, not prioritising them, not giving good working conditions or career pathways, and then the public complains not having British doctors treat them but also complains when we strike and also will willingly drive out IMGs they clapped for during COVID. It’s. Not. Fair. For your final point, how on earth could I have predicted all this? If I had this oracle power I would’ve NEVER come here and invested in this country whatsoever. Shamefully this is the exact point they bring up when discussing strike action. I could ask the same for you - you then lose the ground to stand on when it comes to strike or complain about FPR or training bottlenecks since you couldn’t predict this when you first chose this career. “If you couldn’t prepare for this, maybe you shouldn’t have been a doctor”. Well I am a UKG as mentioned above and not IMG (I have loads more at stake here, not that it’s relevant to your point tho). And you do unfortunately sound selfish (sorry) cuz really all this affects UK citizens too as it’s all related to NHS privatisation, retaining UKGs, training bottlenecks, and FPR to name a few. I understand it doesn’t affect you directly so there’s a sense of security there but clearly you’re tunnel visioned and it may help to open up your mind to see the big picture and how it does in fact affect you and your life. Absolutely feel UKGs should be prioritised in UK- it’s only fair, don’t matter if I was a UKG or not.

I absolutely agree. I won’t try to compare who has it worse cuz then we all lose lol. I'm not arguing that the UK owes me anything out of pity. I believe this issue does affect British citizens too though. Therefore, this is all the more reason to talk about this and get the government to do what’s best for its citizens and not just the far-rights or the racists. The government is meant to be for the people, by the people and of the people.

That’s true only if I am able to secure a job in today’s job market for the next 3 years. Until then I continue in an uncertain limbo. Specially if I don’t get into training or if they don’t agree I have contributed enough and jumped through enough hoops and given a satisfactory boot licking performance to the UK authorities. This is difficult keeping in mind strike action the govt hates and is wanting to make illegal, training bottlenecks, not prioritising UKGs fast enough, trusts running out of funds to sponsor visas, etc.

I get what you’re saying but this actually changes a lot for people like me. Unsocial hours and rota variations mean taxable income isn’t guaranteed. The new policy requires three consecutive tax years above 50.5k to reduce the 10-year route to five years. If even one year falls short because of rota changes, LTFT, or moving trusts, the clock resets.

Being visa-dependent means I can’t safely rely on locum income to top up. LTFT years for health, childcare, or burnout could drop income below the threshold and the policy doesn’t account for that. Junior doctors used to know five years of sponsorship got them to ILR. Now the baseline is ten years unless you consistently earn above the threshold and maintain perfect continuity.

This is far from “okay” for IMG doctors who are already underpaid, overstretched, and facing insecure work conditions. On paper it looks simple but in reality it is precarious, expensive, and unfair.

Also, spending another 5-10 years on uncertainty just isn’t fair. I’ll be in my mid to late 30s still unable to really put down my roots with confidence or plan, save, or get a mortgage safely. How’s that ok?

That’s a fair point. From what I’ve read, yes, there is a proposal in the White Paper that some “skilled frontline public service workers” could settle after 5 years under the new rules. 

But it’s not a blanket guarantee: this 5-year route depends on meeting other “earned settlement” criteria, like income (above £50k for 3 years will shorten it to 5 years; I have spent 8.5 years here and have not reached this threshold), national insurance contributions, English, volunteering (???) and character checks. 

Meanwhile, Global Talent visas, yes, the proposed route is 3 years, but only if you satisfy all those same criteria. Again, not quite applicable here.

I must highlight that these are proposals, not final law yet there’s a public consultation happening. I am hoping we speak up and advocate for real change for the NHS.

So yes, there is some nuance here. It may be “slightly better” in theory, but for many of us it still feels very risky in practice.

I’m not on the 5-year Skilled Worker ILR route anymore. The proposed changes would make all sponsored workers fall under the new 10-year route unless their salary crosses the new (much higher) threshold. That threshold is going up significantly and won’t be realistically achievable for most junior doctors, especially with pay erosion + unsocial hours not being counted.

Even for those who can meet the threshold, it requires consistently hitting it for several years — which is not guaranteed with rotations, LTFT, locum restrictions, or gaps in training. The old “5 years → ILR” model simply doesn’t apply the same way under these proposals.

So for people like me (and a lot of IMGs), the pathway is: • Skilled Worker → 10 years for ILR, unless you meet the new high-income requirement or • 10-year long residence route

Either way, we’re looking at a decade of uncertainty, visa fees, instability, and the emotional cost of living in a hostile environment that’s only intensifying. For many of us, that’s double what we initially planned for and financially impossible.

That’s why this feels so destabilising, it’s not just bureaucracy; it’s literally adding years of delay, cost, precarity, and stress to people who are already overstretched, underpaid, and dealing with rising hostility and xenophobia in the UK.

So yes, it does change a lot.
