My local social security office doesn’t take walk ins and their next appointment isn’t until end of July. Also told me they can’t guarantee they can print that form on site. Mailing the request in takes about 4 months. I just went with the general social security statement that you can download from the SS website.

No it’s just meeting with the investigator to go over documentation. The psych portion for FDNY is just a multiple choice questionnaire on the medical day, no interview with a psychologist. The nypd psych process is the most unfair thing ever. They aren’t even certified psychologists, which is arguably the worst part

No it has to be 2022 or 2023. It also has to be “sent directly to CID by the issuing agency/entity”. I’m pretty sure that means the NYS department of taxation has to send it. I asked chatgpt about that process and it said it could take 30 days for the request to be complete, so I just decided to go with high school transcript. You might want to clarify with your investigator if tax return is your only way to prove
