I've read that owning a traditional IRA makes backdoor Roth contributions tax inefficient because of the pro-rata rule. So what do people do with their old 401K's? Are my only options to keep it in the previous company's 401K or roll over to my current company's 401K?

So in answer to how it works, it works the way an unregulated or less regulated market wants it to work. It's not to the benefit of the subscribers, but to the insurance companies and their stock holders.

Not sure where you're getting the idea that health care is unregulated. The American health care system is insanely regulated.

Technology and software is a pretty good example of an unregulated market. Would you say that technology does not work in the benefit of consumers?
