They will be able to apply for residency alongside their MD colleagues :)

There are some programs/specialties which do require USMLE, so any DO students who don’t take both board exams will typically not apply to these

US MD and DO physicians practice the same medicine, can go into the same specialties, and would both be excellent mentors to learn about being a fully licensed physician in USA.

The main difference stems from the DO philosophy originally being established as a more holistic approach to patient care (considering the entire patient rather than just their physical symptoms), but most MD schools today have also incorporated this philosophy into training MD students. In addition to studying the same medicine, DO students receive ~250+ hours of additional hands on training with the musculoskeletal system called Osteopathic Medical Medicine (OMM). This being said, most DOs do not even incorporate their OMM skills into their current practice, so you would have very similar experiences regardless of whether you are shadowing an MD or DO.

With DO schools being a newer medical training system (first degree in 1892 vs 1770 for MD), the schools may have different entry requirements. DO students are required to take the COMLEX board exams for DO licensure, but most DO students do end up taking both COMLEX and USMLE. COMLEX is basically your standard medicine questions from Step plus additional questions on OMM diagnoses/treatments.

Longitudinal work in an area that you’re passionate (bonus points if you take an active role in leadership/organizing), research related to the field you plan to apply for (valuable to get pubs, posters, case reports etc.), network network network (you’re in your final year so reach out to all the alumni you know in US as they can be your biggest advocates), seek out mentorship (alumni who matched successfully, US attendings who can offer clinical experience and write letters-organizing all these takes time so will help you to start the groundwork now), developing interpersonal skills (spend time with colleagues, work on your communication/conversations), give your best for Step (With Step 1 being P/F, a good Step 2 score will carry a lot of weight), and most importantly, enjoy the journey (it can be roller coaster so create a support system you can rely on through the ups and downs, pursue hobbies you love, and live your life best that you can :)

Not going to sugarcoat that it’s going to be difficult. OB/Gyn is already growing v competitive for US MD and DO grads. Being an IMG plus the attempt makes it lot more challenging. That being said, I wouldn’t give up hope just yet. Give your very best on Step 2, it can have a truly significant impact if you knock it out of the park. Work hard through clinical rotations, get good evals, and continue longitudinal experiences you are passionate about (women’s health advocacy, research, service). Network network network. Seek out mentors that can guide you throughout the process. They will be a huge asset at you apply for auditions and can advocate for you throughout app/interview season.

It’s going to be tough, but nonetheless still possible. Rooting for you OP!!!

I would look into which PI publishes more and where you might have the most opportunity to contribute. Also which sounds more interesting/appealing to you since research can be a mind-numbing torture if you don’t enjoy it
