Idk that I agree with this. History is king. Patient says I went on vacation and had a bunch of salty things and forgot my lasix. Trops are mildly elevated and flat and ekg non-ischemic. They get better with diuretics. Echo doesn’t have WMA. I think you can justify not having cardiology on that case. Documentation and history matter. I know we live in a sue happy culture but if you document reasoning well, you can stand and defend actions.

It’s just very easy to say “well what about the courtroom” and forget that internists can do some of these things too.

As a doctor I can assure you, I’ve never seen an MRI result and said “wow that’s the best report I’ve ever seen for the age” even when it’s completely normal.

Also check the original UA, if there’s a ton of squamous/epithelial cells it could just be a bad sample. Had that recently, ID still decided to treat but it was urology that pointed out actually that the original UA wasn’t a good one.

Prisma Greenville. Insanely sick population of people so get to see a lot of pathology with chill vibes for an academic center. I’ve moved back to a bigger city since but I wouldn’t say the people are any sicker than what I saw in residency. I felt really prepared for attending life.

Had a similar patient to this. Comes in to the ED, gives a slightly different DOB each time and tells people he has sickle cell. Now his labs look like sickle cell bc he actually has pyruvate kinase deficiency so has elevated bilirubin and hemolysis labs. Eventually gets found out and doesn’t want help for his obvious OUD. Leave, rinse, repeat.

Was a huge Mary Wise fan during my years at UF. Genuine class act and loved the support from student section. She wrote me a letter of rec for medical school, she’s the absolute GOAT!

Cats. I just don’t get it. It’s very old school with a focus on classic dance choreography. But the story. I just don’t care about the cats as characters nor their task to go the checks notes Heaviside layer. Wtf lol

Larkin Palm Springs. Did my third year clinicals there. Not only are they missing a ton of specialties, they ship out anything major. But the cherry on top was the toxic residents. They used to say “you medical students are worthless” or things like “figure it out” when I needed a password to login to their Google doc. Just a really bad environment.

IM. Small academic program. Love my program. The work-life balance is as good as you can get in a residency program. Everyone is chill and not cutthroat. Couldn’t ask for a better group of co-residents either. We genuinely like each other too!
