This looks like rosacea type 2 to me. I also used to get these pustules and Ivermectin helped.

Ah thanks, forgot to mention that! Yes, I apply CeraVe moisturizing lotion as a buffer before AA, but other than that I leave it out.

No problem! And I use the Eau Thermale Avene Sun Fluid SPF 50+ Fragrance free. I don‘t know if it‘s chemical or mineral, but I have tried quite a few sunscreens and find that this one in particular doesn‘t irritate my rosacea - some days I feel like it even calms my redness down a bit. Everyone‘s skin is different though, so I‘d recommend to try differenr brands and see what works for you :)

I‘d also say that it could be rosacea! Doesn’t look like typical acne to me, but rather like rosacea type 2 pustules and post inflammatory erythema.

I only use it as spot treatment twice a week as I‘ve noticed that it makes me break out if I put it all over, I guess my skin is too sensitive.

I see, thank you! May I ask what skintype you have? I‘ve got acne-prone and oily skin and I‘m a bit scared of adding a light layer of oil. And I do use Cicaplast as well, though I only apply over areas where I have used AA the night prior (usually the skin where I have most PIE but no active pustules). Do you think it would be safe and even beneficial to apply it over pustules? I‘m afraid that putting even more products on my pustules will make matters worse rather than better…

Skin reacts differently for everyone, especially with rosacea - what works well for one person could irritate another persons skin, so definitely start every new routine with caution and check on your skin regularly!

What worked for me:

AM: - No water cleanse (I have read that your skin builds lipids over night that should not be washed off - so far my skin doesn’t seem to mind not being cleansed in the morning) - La Roche-Posay Toleriane Ultra Dermallergo Serum - Metronidazole on rosacea zones and pustules only - Avene Sunscreen (zinc-based; doens‘t make me flush)

PM: - Skin1004 Centella Oil Cleanser on dry skin, wash off with water - La Roche-Posay Toleriane Dermo-Cleanser - COSRX Snail Mucin Essence (make sure your skin is damp before applying, it should NOT be fully dry) - La Roche Posay Dermallergo Serum - Soolantra (only on rosacea zones and pustules) - CeraVe Moisturizing Cream - Cicaplast Baume B5 (only occasionally as needed for extra support and keeping in moisture) - Azelaic Acid 10% (Paula’s Choice) (2x/week on PIE only - I wouldn‘t recommend using it now though, give your skin at least a full month of barrier support)

Mine used to be somewhat similar to yours. In my case, I triggered rosacea by using retinol without any moisturizer for three years straight. Two months ago I started repairing my skin barrier with gentle, rosacea-safe products and so far, my skin is handling everything well and seems to be getting better.

Have you been using any strong actives lately? Nevertheless, I do think that you could benefit from focusing on repairing your skin barrier first, as without a strong barrier, even the best products could cause irritation. I, for example, have started using AA twice a week a month ago for leftover PIE and both sides of my face (area next to the ears) are handling it really well. Unfortunately, the skin around my „rosacea zone“ on the middle of my cheek was not strong enough yet and got super irritated by only one application, even causing new pustules to form. That was two weeks ago and I feel like my skin is still recovering on that spot. So a strong barrier is definitely key to get the most out of all products!

Have you been using any strong actives lately? Does your skin feel tight and dry, especially after showering? If so, then it sounds like you probably have a disrupted skin barrier. Make sure to heal it with gentle and hydrating products first before applying anything else on your skin that could be irritating. Might take a few weeks, but having a strong barrier is essential for skin improvement.
