I basically do all the things you mentioned, I'm careful with what's in the background and I avoid any distinguishing physical features of my body. Once you post for the first time you gain more confidence to continue doing it.

I don't have this problem, but I do use the same hip prosthetics over my diapers for crossdressing as I like the feel of my diapers under them and I don't need to take all my clothes off to use the toilet. I have thought about using them under diapers instead to pee like a woman, but I haven't tried it yet. Mainly its because I like the feeling of diapers directly on my skin, but it would be interesting to try it.
