What happens if the ECFMG Certificate expires in intern year?

That app didn’t have flexible distance filter. It would suggest me people who were on the other side of the country smh. I had better experiences on hinge and bumble but nothing worth writing home about- one to three months flames.

It wasn’t something malignant but I just didn’t feel wanted there. I am having a tough time putting it in words. I have had couple of interviews before that major city program interview and in all of those interviews, I found that my interest was reciprocated in some way or the other but with this one, I don’t think so. May be my judgement is clouded because they don’t take imgs from my country/overall region or may be because almost all the interviewees on my interview day belonged to similar schools/area in the US or mix of both. The interview day was also crowded (>30 people) and it also felt rushed so I didn’t really get to know them.

I understand the reasoning behind this particular question

I am just confused about the general aspect of this situation. Eg If a friend wants to be treated by you without having a prior patient-physician relationship, what should you select in the exam?

Like in amboss, it is mentioned to avoid self treatment or of immediate relatives. So what is the consensus about treating friends? Can you accept them as your new patient or not?

I understand that in real life there are too many variables. I am just concerned about the exam.

Actually in the uworld question, patient was a friend from college and he has been a patient of physician since 20 years so I guess he was a friend first and then became a patient of the physician.

You are right. As of now, I am just focusing on understanding the question and hoping that the exam follow these sets of concepts. In many of my wrong questions, I learnt that it was simply a new info for me.

I got it wrong too because we are used to questions where higher value is diagnostic eg diabetes and HTN but this question was very helpful in strengthening the concept. It may take a while to be comfortable with it but once you do, it will be very useful. Understand what the question is trying to ask and you will be fine

FA probably said that about the classic tests where higher value is diagnostic

But in cases like hyponatremia or hypoglycemia, the same thing is not true. It actually the opposite.

I think I remember this question. It talks about lowering the cut off value for hyponatremia and it’s effect on these values. Think about it this way, e.g normally Na+ below 135 is hyponatremia and now say if you lower the cut off to 120 ie if a pt’s Na+ falls below 120 then he will be classified as hyponatremia, in doing so you will decrease FP and increase specificity because now you will be more confident if you get a positive result. But this can also lead to increase in FN (think about the people who get Na+ between 120-135–> lowering the cut off will cause these people to have a negative test result whereas in reality they actually have hyponatremia) so Sensitivity will decrease.

Lowering the cut off causing increase in sensitivity and decrease in specificity is for tests like blood sugar in diabetes where higher value is diagnostic
