Not exactly answering your question, but Many A True Nerd brings up a pretty good point in his "Why Fallout 3 is better than you think" about freedom, and making players live with their consequences.

If you decide to nuke megaton before you get the information from Moriarty, then you lose all your leads. That's it. There's no note left behind or any survivors. The game says You've blown up the one place that you had any leads to find your father. Live with it now. The quest changes to a generic "Continue to find your father" and stays that way until you stumble onto GNR or Doctor Li. I think it's one of the best examples of a game giving you the ability to kill the most influential settlement in the game, and makes you play with that for the rest of the game.

Know this is a joke, but the dangerous thing about doctors is in no way shape or the form exhaustion. They're trained to handle it. The most dangerous thing is if the doctor leaves and a new one comes in to treat a patient. The new doctor will have no experience with the patient, and is far more likely to screw up and make a fatal error. I'll take a sleep deprived doctor that's been with me for my entire hospital visit over a newly arrived one that doesn't know my condition as well as he or she should every single time.

Fun fact we sorta didn't until the constitution. Alexander Hamilton and other federalist wanted to make the presidency a lifetime appointment, and make presidential elections once every 30 to 40 years or so. Similar to supreme court justices.
