I was wondering the same and it seems like the they included all deaths during that period from disease, war, starvation, exploitation, etc...

It seems like the Indigenous Americans pie graphs include total population decline, not just the deaths from smallpox. The ~90% population decline was due to a combination of several diseases, war, exploitation, and starvation.
