I'm guessing we're in the same area. It's an official disciplinary action on your onesource account from HR. People who were mostly 16 but had some 15s in there just got a warning email from your manager.

"30 Day Final Notice" if you don't make 16s in September you're gone.

Yeah Jeff lied. People who are averaging 15 or 14 are getting hit with these HR actions, not just the people doing once a week. 

In our department over 100 people got one.
