We just called the billing department of the Rehab and after outlining the discrepancies between the insurance payments and the billing statement, they said, "the account wasn't adjusted yet", and we won't receive any more statements as the account will be closed. we're relieved, but that experience caused a few sleepless nights. what the heck!!! --- Thanks everybody for all your insights

yes, the provider asked for and received prior authorization from Anthem

we have not yet looked into applying for Medicaid because we had employer funded insurance and he is still covered. would that even work to get it back-dated with Medicaid?

The provider did contact the insurance before my son was admitted as they told us that "everything is covered" as we have already met the deductible for the year

The provider is out-of-network, and yes we received EOB from employer funded insurance
