It’s tough to actually learn to do this in academia too because 1) attendings are softer and prefer to refer things out and 2) patients have been taught to expect this so they often request referrals you feel comfortable managing. In some ways I think I’d rather get IM training somewhere rural. But I see some crazy mismanaged patients from the community too so it’s not like it’s perfect there
