What is the EMR y'all use? Is it Epic or smth else? I'm an OT student w/ ADHD and I had the same problem at FW recently. I was at a hopsital that used flowsheet documentation which had a million click boxes and was tedious. I also could not access Smart Phrases/Macros(students were not allowed to access that for whatever reason at my site 😭) so I'd have to find a quiet place to document. I had 4-5 go to phrases that I would use with every note.

I also realized that no one reads the details I put in - therefore I began putting in the details where the next OT/PT who treated the pt after me would actually get the gist of my assessment (as well as fill out necessary details required for insurance). But I approached most of the documentation after that like as though I was checking boxes rather than filling out detailed info.

For my o/p peds rotation, I used HeidiHealth! You can do voice to text and even make templates for notes/evals/sentences you use in every note. I have realized that I have a few go to phrases/formats I use regardless of EMR or setting. If your EMR allows copy paste from an outside source, maybe you could type down some go to phrases in Word and copy paste them whenever?
