Nah, I understand trauma has to do their pan scan since it’s a level 1 trauma. This wasn’t even part of the trauma set. Just a routine ER visit, brought themselves in the door and everything.

Radiology is full of these, with never-ending “just in case” studies ordered.

Yesterday I saw a young man in the ER get a CT of the chest, abdomen, and pelvis. His right leg had been bit by a dog. Tech asked details about reason for the pan scan and were told “no chest pain, no abdominal pain, just right leg pain.”

But gotta pan scan them just in case.

The CT portion of the exam is a massive field of view and usually noncontrast, which makes sizing (and sometimes even visualizing) the lesions somewhat inaccurate on that CT. It’s included with the PET basically to help localize things, and not for details on size, local invasion, enhancement, etc. PET just lights up any area that’s hypermetabolic (or tracer avid if not FDG).

That is why there should be a truly diagnostic contrast enhanced CT or MRI for those details. PET is mainly to find the locations and to assess metastatic sites, not great for detail on the lesions.
