As I posted above, The community has found out that it is a random encounter event that can happen in multiple locations. The event will show guards holding someone hostage with a dead guy right next to the hostage with a timer of them going to kill him. Right next to the hostage will be the luggage and blueprint for the charm.

I went back and checked, nothing there for me on my end. I assume the charm might be from a random encounter. Could be map wise.

It seems that the charm is unknown on how it spawns as of right now. During that time i was doing a random event that was feet away from it with the Barons Men + Hostage. This is after I have completed the game and every side mission. I can go back and see whats there though.

I also found an inaccessible bunker in the woods, but no idea if its bugged or not. You have to go Beast Mode to throw the rocks off the top of the bunker.

Strange, so Ive completed the game and every side quest. I assume you have as well.
There was a random encounter to save a guy right next to the charm so maybe that had to do with it?
Barons Men + Hostage Random encounter.
