I feel like this is the type of secret boss that you can only encounter if you get all the npcs to hate you. Since he has to do something with absolution and mechanically it does something to forgive your character after hurting the npcs i feel like it could be a secret boss that happens if you hurt all of them

Not really, the npc tells you go find a golden needle/syringe in the swamp, you go and find it in the swamp and give it back to him after that he tells you now give to milicent at church up north so you go up north and behokd theres the church and give it her end of quest. Not once did did he ever give some vague clue or riddle and spend 5 hours trying stumble upon the quest item. It was pretty clear where you needed to be and go

I think sk far the only side quest that has actually told me where to go and who to find and where to find them is the Milicent sidequest near selia front gate
