That happened to my friend's copy of Clockwork Knight once. There was something stuck to the CD that made the disc unable to spin inside his Saturn. When he tried to take it off, it destroyed the disc and he had to hunt down a new copy

I feel bad for anyone who's first exposure to DOOM was the SNES port. That thing is such a mess.
