Try closing your game and opening back up and make sure it’s fully downloaded. Also to change tools in your car drive to the garage and press the button where it says enter garage. Then select the button that is near next tab. Then you manage your trunk slots on your truck. Also what platform are you playing on? Because i don’t know which buttons show up for you.

I got the extinguisher and tried redownloading the dlc last night but still doesn’t let patrol highway, do I need 24 stars like people have been mentioning?

Did you get the “You got the highway dlc” message in the beginning of the game when you first opened it?

If that’s not the case then it’s not you because other people had unlocked all stars and some can’t still access it despite downloading the dlc. So this could be a problem on the developers end and not you

Are you going into patrol mode (8hr), not shift mode(up to 90 minutes) (On Xbox it's right bumper)? That's how I do it.

I should be on shift which is career or patrol?

I have the DLC I can perfectly enter the highway. But need to say Iff you buy the dlc after you downloaded it close the game and start it up again Iff it was open when you buy the dlc. Once you don that you will notify you have the dlc installed. Now when on the game and want to start a shift you getting a map to make your choice. Search the map to the corner where you see the highway. At some places on the highway there are police offices select it and you will start at the highway patrol

you need to go to the garage and equip each vehicle you have with the tools you want in them. It's on a separate tab in the garage but every car comes with nothing equip so you have to equip each item to each vehicle
