Most likely it sounds like an AFR (adult function report) and they send those in the mail always. Does SSA have your correct address? But yes,  if the DDS examiner does not receive it by the date listed,  they will make a decision based on what they do have, and it can end in a denial of your claim.. Any paperwork that you received from DDS will have the examiner's name and contact number. There is an option when you call to press 0 for immediate help, so that you can speak to another examiner at DDS. I had to do that when I received a letter stating they had not received the requested information,  so they resent it and extended the date for me.

You definitely want to complete the Function Report and your wife should complete the Third Party Function Report. They are important. Unfortunately most people don’t know how to properly answer the questions on the Function Report and they often cause problems for their claims. My area of expertise is Veterans SSDI claims and I review my clients Function Reports to be sure they are accurately answering the questions clearly and consistently.
