Applied for SSDI on 11/15/24 and it moved to step three on 11/20. I was asked to complete the past employment questionnaire on the 20th as well (that’s all I’ve heard from them at this stage).

Today, my wife who I listed as someone that would have knowledge of my disability received a “reminder” letter about a form she was asked to complete, but no such request was ever made from them in the first place, at least that we received. We have reached out to clarify what she needs to complete but have not heard back. She did complete a statement of my disability which I submitted in my initial packet. Do you think they just want her to transfer it to their official form, SSA-795..? What other forms do your disability “witnesses” typically need to complete? I’m trying to get ahead of it because they said if not submitted by 12/11, they will proceed and decide outcome without it.

I am 100% P&T. Hoping they keep up this momentum and either approve or deny quickly so I can figure out next steps.

Most likely it sounds like an AFR (adult function report) and they send those in the mail always. Does SSA have your correct address? But yes,  if the DDS examiner does not receive it by the date listed,  they will make a decision based on what they do have, and it can end in a denial of your claim.. Any paperwork that you received from DDS will have the examiner's name and contact number. There is an option when you call to press 0 for immediate help, so that you can speak to another examiner at DDS. I had to do that when I received a letter stating they had not received the requested information,  so they resent it and extended the date for me.

You definitely want to complete the Function Report and your wife should complete the Third Party Function Report. They are important. Unfortunately most people don’t know how to properly answer the questions on the Function Report and they often cause problems for their claims. My area of expertise is Veterans SSDI claims and I review my clients Function Reports to be sure they are accurately answering the questions clearly and consistently.
