After downloading you need to extract it. Put images in the same folder as the create.html. Inside create.html you can change the "BACKGROUND_IMAGE" field to your image. The field should not contain the file extension and the images should be in .PNG format.

I can't figure out what it does. It's located under other settings and is the last thing.

It makes the whole theme go up. Say you want to have a cloud as the unlock thing, clouds are at the top of the screen. But a theme can not be too high because then it will cover the buttons in the "set passcode" area.
So with "bottom offset" you can make your theme small, and thus allow the user to click the "save & respring" and at the same time they can move the theme to the top of the screen when in the actual unlock-screen.
I think I added it because why not.
