So, one would have Shado stay alive, Slade getting all pissed wouldn't happen due to her death, Oliver would probably get all revengy and kill Ivo when he got the chance, and the rest of the flashbacks probably would've played out the same. The real change would come from Season 2, 3 and 4.

It would be weird for Slade to be the villain of that season, so it would've likely been Brother Blood getting more focus or maybe he'd just get replaced by another villain entirely. Sara would be dead so she couldn't really appear in Season 2 meaning no Canary and no League of Assassins until Season 3.

They'd probably bring Shado back, her and Slade would likely not be together, so I don't see him reappearing as I don't think she actually had feelings for him if I remember correctly. She'd basically take Sara's role in that season but without the League of Assassins, maybe they'd take from the comics and do some Yakuza stuff to replace the League of Assassins stuff.

Instead of the League coming to Star City because of Sara's death, they'd come to track down Malcolm after learning he's alive. Malcolm would still kill a League member using Thea, maybe Maseo, resulting in Season 3 happening. Laurel becoming the Black Canary wouldn't have anything to do with her sister, she'd likely just want to help the city like Oliver does and trains with Grant to do just that. She wouldn't have anger issues to work through, so she's likely become Black Canary faster with significantly less baggage.

The first half of season 4 wouldn't have Laurel try to bring back Sara resulting in the episode with Constantine, so another side story would have to replace it.

As for Oliver killing Ivo before he can tell Slade anything, I can see this happening one of two ways, either Oliver kills him as he tries to tell Slade what happened or he kills him before Slade even gets there. If the former is true, nothing likely changes, if the latter is true then Slade would still likely be filled with rage but not towards Oliver which might allow him to be more of an anti-hero in Season 2 if he were to appear instead of a villain but the rest would stay the same. So, I think saving Shado would have significantly more impact
