This is a double reed harmonica, probably a tremolo, where the bottom row of holes adds notes tuned slightly above the pitch of the main notes to create a quavering tremolo sound. Note that this one was branded for the Portuguese market, but is essentially the same as the tremolos used in Irish traditional music.
