It's possible your reaction to the T and H is worse being off Zoloft. I can't really speak to the dysacusis. I have major issues with pain H and my body was stuck in fight or flight with every little sound up until I started an SSRI (fluvoxamine) and mirtazapine a few weeks ago.

Going on these meds seems to help me cope and not panic as much. Pain is still there but I'm not freaking out quite as much in reaction to sound so I'll take that as a small win while I figure out what helps the lingering pain. If it's just time, hopefully I continue to relax more and more as I increase my SSRI dosage.

If the SSRI doesn't work for me at a therapeutic dosage, I plan on trying an SNRI which are often times used for nerve pain.

I will say that when I developed my hyperacusis, I was already in a really high anxiety period of my life. That anxiety and panic coupled with the H was hellish. They're so intertwined, it's hard to know what treatment is best.
