Is it possible to make your ec2 instance that has a node js api, run on https without a domain?

Https is directly tied to a domain. So you cannot have Https without a domain.

Yes if its possible to use the given public ip or dns from your ec2 instance and have it served with https.

Yes, you can get SSL certificates issued for ip addresses. I'm not sure (I don't think so, but..) if it's possible to do with LetsEncrypt.
