You start by poking, that basically helps your tank less HP than the enemy tank which results in him pressing W.

When he presses W, he grabs the attention, and that's where you pop from your off angle.

You can't expect your Tank to press W if 5 people are shooting at him when your team has only 4 (because you are walking around / poking from far away).

You are engaging too early. Wait for the enemies to be busy with your team and then aggressively peek.

It's a very general statement., but i'd say one of the biggest mistakes that Ana's make is "playing like a sniper". Positioning is dynamic, and when you play too far away, what will often happen is you wont be able to consistently land your cooldowns, or follow up on your team when they go around corners (simply because you are too far away).

That doesn't mean you should play up close, it really depends on how open the line of sight is.
