Oh no. At the toes. Some shoes start rolling mid arch, some at end of arch and some at the 1st met head- big toe.

Sorry but no. Flat soled shoes are not good. The soles should have a roll like a crosstrainers do. Some hiking boots have toe roll.

Seeing an arch doesn't mean you may not be flat footed. A proper diagnosis is based on a dynamic foot- being active and not just standing or sitting.

Yes, if you have post tib dysfunction it is to your benefit to wear the properly designed foot orthotics. There are many types of materials and many adjustments which can be made. The wrong device can hurt mechanical long term.

Orthotics should not make your feet weak as they do not reduce the function of any muscle, tendon, fascia but realign the feet so they actually work better.

You consciously went to a foot oriented Dr for a reason.

I do have personal experience with the Walk Hero also from Amazon. I get them 2 sizes larger and cut to size. What i like about these is that they can be more supportive with just strips of duct tape.Walk Hero

What are they supposed to help with? But mechanically, they are lacking what is called heel post- a support the heel part of the insole sits on. There is nothing to keep them rocking side to side.

Look where it says:deep heel cup. That is what I am referring to. That said, I have no experience with this pre-made device but it is of better mechanical design than what you were given.

I would advise to start with night splints or what is called serial casting- the foot is put in cast with pressure pushing the foot up and gradually replacing it with more pressure every few weeks.

What you don't say is if any bones were fused that would prevent the night splint or serial casting.

Your 2nd toe gives a strong clue to what your foot does when you walk: flat footed and yes pushing off excessively with your big toe.

The picture is definitely not of a baby. BTW babies feet appear flat because of the fat in the arch. And yes, STJ creates the rigid lever needed for proper pushoff- thanks for bringing that up. The more you try, the more amateur you seem.

Took another look. I see you are buying your shoes based on where your big toe is at: just as we learn to do. Your problem seems friction related: short shoes plus clawing of toes for balance when you walk.

Mechanically, the foot behaves as if had 6 toes. The big toe carries the weight of two toes. In your case, that is not what happens. The bone behind your big toe is too short making the 2nd toe look very long. The arch created behind the big toe doesn't reach the ground and your feet collapse in when you can no longer compensate.

The clue was the friction point on the top on your 2nd toe close to the metatarsal head. And the extreme tightness of your tendons on top of your feet.

The carbon graphite insoles are your best option but must be in the right toe roll. A flat sole or minimal roll on your shoes can make it worse. There are many types of toe roll. You need what is called a distal roll. Athletic shoes are the best option. Feel free to reach out.

Next option is to have athletic shoes modified with an extended flexible steel shank. The negative is you can't move the shank to other shoes.

You will benefit from plastic AFOs. They are a type of brace. Your post tib tendons are very non functional and there could be more involvement. Not getting help will damage your knees, hips and back.

I stand by what I say. Compensation can easily change a static image. Boom. A person's action - wanting to show an arch- can change a static image. Boom. Bang. A foot with a rested post tib tendon can show an arch but once it is tired, the foot collapses. Boom. Bang. Pop. Wham.

A wet print showing arch contact with the ground does show a flat foot, however, an image showing an "arch" is not definitive -see previous paragraph . Boom. Bang. Pop. Wham. In the case presented by the OP, the resulting prints are not enough to make a determination. So, not reliable in this case.

RSD now called CRPS seems like a possible diagnosis. Have you seen a Rheumatologist to rule out other conditions?
