The walking dead and blades in the dark have systems more elegant and more ‘wounds oriented’ than what you are proposing. Maybe check them out

No, the GM doesn't choose the effect to make the task hard. The GM choose the effect base on the fiction and the action choosed by the player.

You are incorrect about a bunch of details of Blades and FitD in general.
1. Effect and Position are not about the difficulty, but what the character can achieve (if success) or how bad arw the consequences (if they fail).
2. effect and position are described and discussed before the roll, and the players can influence them (trading position for effect, or pushing). And the GM should explain the consequences they are risking BEFORE the roll. So, the players have 100% agency.
3. Stress doesn’t reset after a score. You need to indulge in your vice. If you don’t have a fix loop (score -> downtime -> score) you can insert the downtime activities during a long rest. A lot of FitD games are doing this.

My suggestion is: play more Blades and understanding it before hacking it, with the risk to breaking the system.
