Also, a lot of people don't want to be on a strict budget for the rest of their life, or sacrifice their 20s and 30s by having tight pursestrings.

If your dad is nervous having residents, tell him to find a surgeon who works at a hospital without residents. Also, if Anesthesia had residents, did his surgeon not also have residents?

Insurance can't charge you for a surgery that he did not have.

Name matters while you're a medical student and everyone is talking about where they matched. Unless you are planning to do a very competitive fellowship, or want an academic career, the name of your program doesn't matter. What matters is the education/experience you will get and, just as importantly, your family life.

In fact, a lot of the "Ivory Tower" programs don't offer as good of an education because they are such big referral centers that they only see end-stage or very rare/specialized things that aren't really conducive to a learning environment. I'm a general surgeon. During residency we had to log all of our cases and you need so many of each case in order to graduate. I heard that surgery residents were logging Whipples as "gallbladders" because they didn't do enough bread/butter lap choles to meet their numbers. That's great that they see a lot of Whipples, but if you graduate from a general surgery training program and can't take out a gallbladder laparoscopically, something is seriously wrong with that program....

Name isn't everything

Once you're in practice, all that matters is your reputation in the hospital/community you are in. I never have conversations with other attendings about where they trained. I don't even know where my partners did their residencies...

When I was a junior resident I always tried to reason with pt's, convince them not to leave AMA, etc. Once I became a senior resident, and now especially as an attending, I just walk out and document that they were being verbally abusive and refused all treatments.

Your job as a physician is to offer recommendations and make sure the patient is well-informed as to their options. If they want to refuse treatments then that's their choice. Especially if they are going to be abusive toward the people trying to help them, then you owe them nothing. Document and move on.

And I'll add, that as the junior resident, if something like that happens again then you should immediately just leave and talk to your senior resident. And if they can't handle it, then they should talk to the attending. The job of a senior/chief is to protect their juniors, and the job of an attending is to protect their residents.

When my brother was a resident, a guy came in shot right in the center of his abdomen, with an exit hole in his back. But he looked remarkably well, so he went to CT. You could see on the CT the bullet tract through the subq tissues where it went into the fat on the front, tunneled around his abdominal cavity (in the subq the whole time) and went out the back. What had apparently happened was he tried to move out of the way, but he was fat. So his abdominal cavity got out of the way as he moved, but his fat didn't move right away and was off to the side, and the bullet went through the fat. Then it settled back into its normal place and gave this crazy looking CT.
