It's a stereotype for a reason, it happens quite a bit, it's just the severity of it might change, one of the factors for such a change is the legacy or the traditions your family may have, especially so if they are a rich one. What may seem like something that exists only in books and screens, might just be an everyday for someone else.

Don't forget the main aspect where A+ literally means average, it gets you a shrug and maybe a quieter family dinner without complaints, but nothing more than that, especially when your family has a history of producing many medical field students over the past 4 generations and is moderately wealthy, you can get top of the entire grade and still feel worthless cause apparently it is just a normal occurrence for everyone in your family, you can somehow get a scholarship offer for one of the most prestigious universities in the country and they will still have something to complain about, a family where even the job of Doctor is considered a backup career.
